Study Of Analysis Of Spiritual Needs And Effects Of Dignity Therapy On Hematologic Malignancy Patients | | Posted on:2019-10-10 | Degree:Master | Type:Thesis | | Country:China | Candidate:J Y Chen | Full Text:PDF | | GTID:2404330569981138 | Subject:Nursing | | Abstract/Summary: | PDF Full Text Request | | Objectives 1.Systematically review spiritual care interventions and its effects on quality of life and spiritual well-being among patients with terminal illness.2.Describe the spiritual needs of hematologic malignancy patients.Analyze the influence factors of spiritual needs among hematologic malignancy patients.3.Assessing the effects of dignity therapy on hematologic malignancy patients.Methods 1.Systematically search studies of RCT and CCT about the effects of spiritual care on terminal patients in EMbase、MEDLINE、CINAHL、AMED、PsycINFO、CENTRAL、CBM、CNKI 和 Wanfang databases.Search time-limit is from building databases to May 2017.Two researchers screened the literature,extracted information and assessed the quality of literature independently.2.This study is a descriptive research.Collecting data of 253 hematologic malignancy patients using SNAP questionnaire in three different hospitals from March 2017 to June 2017.All data were analyzed with descriptive analysis,t test,ANOVA and multiple regression analysis.3.A randomized controlled trail was conducted(N=66).The participants were allocated to the experimental group(N=32)or the conrol group(N=34)according to the random number table.The subjects in the experimental group received dignity therapy.Data were collected at baseline,immediately after intervention and 1month after intervention with FACIT-Sp-12,Herth Hope Index and EORTC QLQ-C30.The data were analyzed using ITT analysis and PP analysis.Results 1.Sixteen studies with 1209 patients were identified.Six different spiritual care interventions were included in this systematic review.Narrative analysis showed that spiritual care was beneficial for global quality of life of patients with terminal illness.Narrative spiritual care interventions were more beneficial for spiritual well-being among patients with terminal illness.Meta-analysis showed that spiritual care could improve the social function[SMD=0.75,95%CI(0.38,1.12),p﹤0.0001] and emotional function[SMD=1.13,95%CI(0.40,1.85),p=0.002] of terminal patients.2.Most hematologic malignancy patients have spiritual needs.The highest five scores of items were psycho-social dimension.The lowest five scores of items were religion dimension.Multiple regression analysis showed that degree of education,religious faith and recurrent patients were the influence factors of spiritual needs of hematologic malignancy patients.3.ITT analysis and PP analysis both showed that the scores of emotional function,social function,spiritual needs and hope level of experimental group were higher than that of control group after intervention(p﹤0.05).Conclusions 1.Spiritual care is beneficial for global quality of life among patients with terminal illness.Narrative spiritual intervention is more beneficial for spiritual well-being among patients with terminal illness.2.Most hematologic malignancy patients have spiritual needs.Health care professionals should pay more attention on patients’ spiritual needs.3.Dignity therapy is beneficial for short-term emotional function,social function,spiritual well-being and hope level among patients with hematologic malignancy.It’s worth promoting the application of dignity therapy in health care settings.It’s long-term effects need more researches to explore. | | Keywords/Search Tags: | hematologic malignancy, spiritual needs, dignity therapy, quality of life, spiritual well-being | PDF Full Text Request | Related items |
