Objective1.To understand the prevalence of irritable bowel syndrome in Yangzhou nurses under the Rome IV diagnostic standard.2.Explore the status quo and influencing factors of the quality of life of nurses with irritable bowel syndrome.3.To explore the influence path of the spiritual climate of working environment on the quality of life of nurses with irritable bowel syndrome.Methods1.From November 2021 to June 2022,according to the inclusion and exclusion criteria,clinical nurses from 7 hospitals in Yangzhou City,Jiangsu Province were selected as the study subjects for cross-sectional investigation and qualitative interviews.2.To understand the prevalence of irritable bowel syndrome among nurses through cross-sectional survey;The quality of life of nurses with irritable bowel syndrome(IBS)and its influencing factors were investigated by comprehensive qualitative and quantitative research methods;According to the theoretical model and the previous research results,construct a hypothesis model,and use the structural equation model to verify the impact path of the spiritual climate of working environment on the quality of life.The main research tools include the self-designed general information questionnaire,irritable bowel syndrome diagnosis questionnaire,irritable bowel syndrome symptom severity questionnaire(IBS-SSS),irritable bowel syndrome patients’ quality of life scale(IBS-QOL),hospital anxiety and depression scale(HADS),Pittsburgh sleep quality index questionnaire(PSQI),Maslach Burnout Inventory(MBI),and spiritual climate simplified scale.3.Excel is used to establish the database,and two people enter the data;SPSS 25.0 software and AMOS 23.0 software were used to analyze the data.The statistical methods included descriptive analysis,independent sample t-test,one-way ANOVA,multiple linear regression,Pearson correlation analysis and structural equation model analysis.Results1.Analysis of the prevalence of irritable bowel syndrome among nurses in YangzhouOf the 1394 clinical nurses,215 met the Rome Ⅳ diagnostic criteria and were diagnosed as IBS.The overall prevalence rate of IBS among nurses was about 15.4%.Among 215 nurses with IBS,114(53.0%)were diarrhea type(IBS-D),43(20%)constipation type(IBS-C),followed by 40(18.6%)atypical type(IBS-U)and 18(8.4%)mixed type(IBS-M).The total score of IBS-SSS of nurses with IBS was(205.92 ± 81.69).More than half of the patients(119,55.3%)had moderate severity,56(26%)had mild severity,and 27(12.6%)had severe severity.2.The quality of life of nurses with irritable bowel syndrome and its influencing factorsQualitative research results:All 8 nurses interviewed believe that irritable bowel syndrome has led to a decline in their quality of life to varying degrees and has affected their work efficiency;The data analysis concluded that the factors affecting the quality of life of nurses with irritable bowel syndrome included two themes,namely,factors related to intestinal symptoms and occupational psychological factors.Factors related to intestinal symptoms include four sub themes:abdominal pain,diarrhea,abdominal discomfort,and dietary restrictions;Occupational psychological related factors include four sub themes:work stress,sleep disorders,fatigue,and the spiritual climate of working environment.Cross-sectional study results:The total score of quality of life of 215 nurses with IBS was(81.59 ± 16.41),significantly lower than that of healthy control group(P<0.001);The results of univariate analysis showed that there were statistical differences in the quality of life scores of nurses with different hospital grades,food and drug allergy history,food types that induced abdominal pain,family history,regularity of three meals,and IBS subtype,and the quality of life scores of nurses with different IBS-SSS,HADS,PSQI,MBI and spiritual climate scale scores also had statistical differences(all P<0.05);The results of multiple linear regression analysis showed that hospital grade,severity of IBS symptoms,anxiety and depression,spiritual climate and job burnout were the factors affecting the quality of life of nurses with IBS(F=1 4.935,P<0.001),and could explain 56.6%of the total variance.3.Analysis of the influence path of spiritual climate of working environment on the quality of life of nurses with irritable bowel syndromeAfter revision,the model of influencing factors on the quality of life of nurses with IBS was well fitted,and the adaptation indicators were as follows:x2=97.032,df=68,x2/df=1.427,GFI=0.941,AGFI=0.909,CFI=0.987,NFI=0.958,IFI=0.987,RMSEA=0.045.The initial hypothesis was established,and there were three significant paths for the impact of spiritual climate of working environment on the quality of life of nurses with IBS:① Spiritual climate→quality of life②Spiritual climate→anxiety and depression → quality of life③Spiritual climate→anxiety and depression→job burnout →quality of lifeConclusions1.The prevalence rate of IBS among nurses in Yangzhou is about 15.4%,which is much higher than that of the general population in China.Most of the patients with IBS among nurses are diarrheal type with severe symptoms.This suggests that medical institutions should pay more attention to nurses with IBS,pay attention to the adverse effects of intestinal symptoms on the work and life of nurses,and actively take measures to help them alleviate symptoms and improve the quality of life,which will also help nurses improve their work efficiency.2.The quality of life of nurses with irritable bowel syndrome has significantly decreased,and the influencing factors include factors related to intestinal symptoms and occupational psychology.Among them,hospital grade,symptom severity,anxiety and depression,occupational burnout,and spiritual climate are independent influencing factors for quality of life.This suggests that nurses themselves and nursing managers should not only pay attention to the adverse effects of intestinal symptoms,but also pay attention to the impact of occupational psychological factors on the quality of life of nurses.This study provides a theoretical basis for finding intervention targets for the quality of life of nurses with irritable bowel syndrome in the future.3.The impact of spiritual climate of working environment on the quality of life of nurses with IBS includes one direct impact path and two indirect impact paths.The structural equation model fits well and clearly shows the mechanism of action between various variables,suggesting that the specific measures to improve the quality of life of nurses with IBS in the future can start with reducing work pressure and improving the spiritual climate of working environment.
