ObjectiveFirst,this study aimed to culturally adapt and test the psychometric properties of the Chinese version of the Gout Impact Scale(GIS)among patients with gout.Then,to investigate the quality of life in gout patients and explore indicators which influence the quality of life in gout patients.MethodsThe Chinese version of the Gout Impact Scale was achieved after undergoing forward and backward translation of the GIS by expert consultation.Item analysis was tested by the critical ration and correlation analysis.Content validity was tested by expert consultation.Construct validity was tested by exploratory factor analysis(EFA).Reliability was tested by using Cronbach’s α,split-half reliability and test-retest reliability.A convenience sample of patients with gout was recruited from two tertiary hospitals in Tianjin.We used the self-designed General Condition Questionnaire,typical gout attack pain-Visual Analog Scale(VAS),patient-rated severity-Visual Analog Scale(VAS),the Chinese version of the GIS,Self-rating anxiety scale(SAS)and Self-rating depression scale(SDS)to investigate the related situation of the patients with gout.T test,one-way AVOVA,rank sum test,Spearman’s correlation analysis and stepwise multiple linear regression analysis were used to analyze the data.Results1.The CR value of the items in each dimension of the scale were greater than 3.000,and the difference was statistically significant(P<0.001),and all 24 items of the scale had good discrimination,so all items of the scale should be retained.The correlation coefficients between each item of the scale and its dimension were 0.659 to 0.934(P<0.01).Both the content validity index of 24 items and the overall scale were 1.00.The exploratory factor analysis of the scale extracted six common factors and its variance contribution rate were equal to or greater than 4.288,which could explain the variation of 77.425% of the total scale.The factor loadings of all items on the common factor of the scale were equal to or greater than 0.411.The internal consistency of the total scale was 0.916 and the Cronbach’s alpha coefficient of the GI scales were sufficient or excellent(0.678 to 0.943).The split-half reliability coefficient of the total scale was 0.726 and the split-half reliability coefficient of the GI scales were sufficient or excellent(0.693 to 0.917).The test-retest reliability coefficient of the Chinese version of the 5 GI scales were 0.731 to 0.844,all of which were greater than 0.700.2.The current status and influencing factors of the health-related quality of life in gout patients: The average score of “gout concern overall” scale was 85.95±17.69.The average score of “gout medication side effects” scale was 75.39±21.20.The average score of “unmet gout treatment need” scale was 35.48±20.07.The average score of “well being during attack”scale was 62.49±21.95.The average score of “gout concern during attack” scale was 68.19±18.95.According to the results of stepwise multiple linear regression analysis in accordance with the order of the regression coefficient from big to small,the influence factors of “gout concern overall” scale were in patients with attack in the past 3 months or not,working or not and household income,which explained 24.9% of the variance of the scale.The influence factors of “gout medication side effects” scale were patients-rated severity VAS and literacy levels of the patients,which explained 15.5% of the variation of the scale.The influence factors of “unmet gout treatment need” scale were attack frequency in the past year,patients-rated severity VAS,the serum uric acid level at present and gout attacks over the past 3 months,which explained 52.1% of the variation of the scale.The influence factors of the “well being during attack” scale were the typical attack pain VAS and working or not,which explained 33.8% of the variation of the scale.The factors influencing the “gout concern during attack” scale were patients-rated severity VAS,anxiety and age,which explained 28.6% of the variation of the scale.ConclusionThe Chinese version of the GIS exhibited acceptable reliability and validity characteristics.The results tested by the scale were reliable and effective.The use value of the Chinese version of the GIS was high,which should be popularized and applied further in patients with gout.The quality of life in gout patients was in the medium to low level.Age,working status,literacy level,household income,patients-rated severity VAS,attack frequency in the past year,the typical attack pain VAS,gout attacks over the past 3 months,the serum uric acid level and anxiety in patients were the main factors that influenced the quality of life in gout patients.Health care workers should pay more attention to the assessment of the quality of life in gout patients and detect the problems in time.Then,effective and comprehensive intervention measures should be taken to improve the quality of life in patients with gout. |
