| Objectives:Epidermal growth factor(EGF),Interleukin-2(IL-2)and Platelet activating factor(PAF)in stress-induced ulcer is currently known about the pathogenesis of an important position,but the three in the stress ulcer in the specific level of change is still not clear.Therefore,this study analyzed the clinical relationship of serum epidermal growth factor,interleukin-2 and platelet activating factor in patients with stress ulcer in ICU.Methods:One hundred thirty-six patients who were treated in ICU from Tianjin Port Hospital from September 2014 to October 2016 were selected.Including 44 cases of traumatic brain injury,severe trauma in 34 cases,24 cases of severe systemic infection,multiple organ dysfunction syndrome in 18 cases,16 cases of cardiovascular and cerebrovascular accident.In the patient’s vital signs under the steady state of endoscopic examination.Sixty-eight patients with stress ulcer were treated as ulcer group and 68 patients without stress ulcer as non-ulcer group.Patients with ulcer group were grouped according to the outcome of the disease,transferred into the common ward divided into survival group(46 cases),patients who died due to a worsening condition were divided into the death group(n = 12).Patients were given venous blood to separate serum into the ICU immediately,3 days,7 days and 14 days.The levels of serum EGF,IL-2 and PAF were measured by ELISA kit.Results:1.In the time of into ICU immediately,3 days,7 days and 14 days,the levels of serum EGF in ulcer group were lower than those in non-ulcer group,The difference was statistically significant(P <0.05).The level of serum EGF was reduced in the ulcer group and reached the lowest level on day 3,which was lower than that into ICU immediately(T = 10.982 P = 0.000).With the progress of time,although serum EGF levels have rebounded,but slow and low amplitude,at 14 days was still low.2.In the time of into ICU immediately,3 days,7 days and 14 days,the serum IL-2in ulcer group was lower than that in non-ulcer group.And the difference was statistically significant(all P <0.05).The levels of serum IL-2 in the ulcer group were significantly lower than those in the non-ulcer group when into ICU immediately,with the progress of time,although IL-2 levels slowly picked up,there was a significant rebound in the 14 th day,but still lower than the non-ulcer group.3.In the time of into ICU immediately,3 days,7 days and 14 days,the level of serum PAF in ulcer group was higher than that in non-ulcer group,the difference was statistically significant(all P <0.05).The level of serum PAF was increased in the ulcer group and reached the peak at the third day after entering the ICU,and was significantly higher than that into ICU immediately(T = 12.092 P =0.000).Followed by a slow fall,but still higher on the 14 th day than in the non-ulcer group.4.Into the ICU immediately,into ICU 3 days,7 days and 14 days,the difference of serum EGF levels between the three groups was statistically significant(P <0.05).The serum EGF content in the normal group was higher than that in the ulcer group and the non ulcer group,and non ulcer group was also higher than ulcer group.The difference were statistically significant(all P <0.05).The level of serum EGF in the ICU group was lower than that in the normal group and decreased on the third day.As the time progressed,although the level of EGF recovered,but slow and low,EGF levels were 14 days still lower than normal group.5.The level of serum EGF and IL-2 in the death group was significantly lower than that in the survivors group(P <0.05).While the serum PAF level was significantly higher than the survival group,the difference was statistically significant(P <005).Conclusions:1.The level of EGF and IL-2 in patients with stress ulcer is reduced,suggesting that the role of two factors with intestinal mucosal protection and immune regulation is inhibited and play a role in the development of stress ulcer.2.Increased levels of serum PAF in patients with stress ulcers may be involved in the development of gastrointestinal mucosal injury. |
