The Correlation Between Supportive Periodontal Therapy And Clinical Efficacy Of Patients With Chronic Periodontitis,a Retrospective Study

Objective: The aim of this study was to determine the relationship between Supportive Periodontal Therapy(SPT)with the progression of chronic periodontitis and long-term stability of the clinical efficacy by comparing the differences of clinical indexes of patients with or without SPT after initial treatment.This aim may be useful for improving the efficacy of treatment.Methods: According to the inclusion and exclusion criteria,120 patients who were diagnosed as moderate-severe chronic periodontitis from 2009 to 2015 at Department of Periodontics,School of Stomatology,China Medical University were included.According to patients whether performed SPT on time or not,patients were divided into non-maintenance group and maintenance group,60 patients in each group.The average observation time was 35 months.The clinical indexes include Probing Depth(PD),Attachment Lose(AL),Bleeding on Probing(BOP),Plaque Index(PLI),number of teeth.The clinical indexes in the first visit(T0),a month after initial treatment(T1)and the last visit(T2/T2')were analyzed by SPSS17.0 to compare the differences of clinical indexes between the two groups.Results: 1.Comparison between two groups: At T0 and T1,there was no significant difference in PD,AL,BOP and PLI between two groups(P>0.05).At T2/T2',the values of PD,AL,BOP,PLI of non-maintenance group were higher than those of maintenance group((P<0.05).2.Comparison within group: Compared with T0,there was a significant decrease in PD,AL,BOP and PLI in two groups at T1(P<0.05).The values of PD,BOP and PLI in T2/T2' increased in both groups compared with those of T1 and the increase of each clinical index in maintenance group was less than that of non-maintenance group(P<0.05).The AL of T2 was higher than that of T1 in non-maintenance group,while the AL of T2' was slightly lower than that of T1 in maintenance group(P<0.05).3.Comparing the changes of different periodontal pockets,we found that there was a significant difference in the pockets with PD>3mm between two groups at T2/T2'(P<0.05),while there was no significant difference in the pockets with PD?3mm(P>0.05).At T2/T2',the proportion of the pockets with PD>3mm in non-maintenance group was higher than that of maintenance group,while the proportion of the shallow pockets with PD?3mm was lower than that of maintenance group(P<0.01).4.The incidence of tooth loss in non-maintenance group was 0.11 tooth per patient per year and0.09 tooth per patient per year in maintenance group(P>0.05).Conclusions: Patients with regular SPT had a better long-term efficacy than those who did not perform regular SPT.For the shallow pocket,during 3-year follow-up period,there was no significant difference in the efficacy with or wthout SPT after initial therapy.For the moderate and deep periodontal pocket,regular SPT can maintain the effect of initial therapy.Periodontal therapy can be more effective in maintaining periodontal health only combining with regular SPT.
