Application Research Of Family-integrated Transition Care In Patients With Cerebral Stroke

Objective:To understand the actual situation of Barthel Index,quality of life,Morisky Medication Adherence Scale-8 and rehabilitation exercise compliance of discharged patients with cerebral stroke;to analyze the feasibility of family-integrated transition care among discharged patients with cerebral stroke and to evaluate the effect of applying family-integrated transition care in those patients.Method:78 patients with stroke who were admitted in Renmin Hospital of Wuhan University from May 2016 to October 2017 were selected by convenience sampling method and objective sampling method divided into control group and experimental group by random number table method,with 40 cases in each group.The patients in the control group received routine neurological health education,while the patients in the experimental group received family-integrated transition care besides routine neurological health education.Family-integrated transition care included team building,family members skills traing,family-integrated guidance for discharged patients and regular visits.The patients,Barthel Index,quality of life,Morisky Medication Adherence Scale-8 and rehabilitation exercise compliance and satisfaction were compared between two groups.Results:Totally 71 patients finished the research,in group comparisons,the scores of Barthel Index,quality of life scale,the physical dimension in quality of life scale,the psychological dimension in quality of life scale,Morisky Medication Adherence Scale-8,rehabilitation exercise compliance and satisfaction in two groups were statistically significantly(p<0.01),while the scores of the environmental dimension in quality of life scale and the social dimension in quality of life scale were not statistically significantly(p>0.05).In the time effect,the scores of Barthel Index,quality of life scale(all dimensions),Morisky Medication Adherence Scale-8 and rehabilitation exercise compliance in two groups were statistically significantly(p<0.01).In the interaction effect between time and group,the scores of Barthel Index,quality of life scale,the physical dimension in quality of life scale,the psychological dimension in quality of life scale,the environmental dimension in quality of life scale,Morisky Medication Adherence Scale-8 and rehabilitation exercise compliance were statistically significantly(p<0.01)except the scores in the social dimension in quality of life scale(p>0.05).Conclusion:The family-integrated transition care can effectively improve the daily,quality of life,compliance and satisfaction of patients with stroke.
