The Establishment And Reliability And Validity Test Of Home Nursing Assessment Scale For Empty Nest Elderly With Chronic Diseases

Objective:Refering to the Omaha System and Nursing Outcomes classification System,to establish a set of home care evaluation scale which is suitable for community empty nesters with Chronic Diseases,at the same time,the validity and reliability of the evaluation scale are tested and verified,which can provide assessment tools for exploring measures to improve the home care of the community empty nesters with chronic disease.Methods:1.Guided by the holistic nursing care and Maslow's hierarchy of needs theory and based the Omaha System and Nursing Outcomes classification,preliminary construct scale items through site survey,literature review,expert interviews and subject panel discussion and filter items initially through consulting some nursing experts.2.Nursing experts who were selected were consulted by Delphi method for two turns.The active coefficient and authoritative coefficient of experts and concentration and harmonious coefficients of experts' opinion were calculated based on the feedback.Applying the "Boundary Value Method" to Screen and Modify Index Items to form formal scale entry.3.345 patients in a community health service center were selected to conduct a preliminary survey.The results were analyzed by SPSS21.0 statistical software.Through the discriminant analysis and reliability analysis method,the items were screened and the remaining items were analyzed to further filter the entries,and the structural validity of the scale was preliminarily determined.The internal consistency alpha coefficient was used to evaluate the reliability test.Validity was evaluated by structural validity.4.539 patients from three community health service centers were selected to conduct a large sample formal investigation.Results SPSS21.0 statistical software and Amos20 software were used to analyze them.The reliability test was also evaluated by the internal consistency ? coefficient.Confirmatory factor analysis was used to evaluate its structure validity to verify whether the structure of the scale of the meter was in accordance with the theoretical conception.Results:1.Expert consultation resultsAfter two rounds of expert consultationand comprehensive expert opinion,we formed the final survey scale which includes 4 areas,24 dimensions,a total of 64 entriesle.The statistical results show that the experts have high enthusiasm and authority.2.Preliminary investigation resultsThrough the distinction analysis,reliability analysis method and exploratory factor analysis,5 entries were deleted.4 areas,24 dimensions,and a total of 64 items were formed,and 24 factors cumulative contribution rate of the was 85.685%.3.Formal investigation resultsThe total Cronbach's ? coefficient of the scale was 0.931,and the result of confirmatory factor analysis shows that,2c /df=1.414,GFI=0.869,AGFI=0.858,IFI=0.961,TLI=0.959,CFI=0.961,RMR=0.066,RMSEA=0.028 were all within acceptable limits.Conclusion:1.Initially,the community empty nest elderly chronic disease home care evaluation scale,including 4 areas,24 dimensions,a total of 64 entries.2.Community empty nest elderly chronic disease home care evaluation scale has good reliability and validity,which can be used as a quantitative tool to evaluate whether the nursing measures of community empty nest elderly chronic disease are in place.
