| Objective: To investigate the syndrome differentiation of type 2 diabetes mellitus and improve the modern biological basis of syndrome type by metabonomics,to investigate the characteristics of retinal blood oxygen saturation in DM patients with different TCM syndrome types.Methods:Through collecting the symptoms and signs of traditional Chinese Medicine of 96 patients with type 2 diabetes mellitus,according to Yan Shilin syndrome differentiation system,obtain the TCM Syndromes of diabetic patients.Based on serum and urine metabonomics,analysis of metabonomics in different TCM Syndromes,improve the modern biological basis of syndrome type.Then the retinal blood oxygen saturation in 96 diabetic patients and 99 normal subjects were measured by retinal oximetry.To compare the difference of retinal blood oxygen saturation between DM patients and normal subjects and investigate the changes of retinal blood oxygen saturation in different TCM Syndromes of DM patients.Results:(1)By means of analysis of TCM syndrome elements of type 2 diabetes mellitus,combine metabonomics analysis of serum and urine in different syndrome types,finally the type 2 diabetic patients can be divided into hyperactivity of fire due to deficiency of yin,heat stagnation of liver and stomach,deficiency of both qi and yin and yin and yang deficiency four TCM syndromes.(2)At last,96 normal persons and 70 diabetics’ retinal oxygen saturations were came out,among which the difference between the men and women、between the different ages、between different course of diabetics have no statistical significance(P >0.05).The retinal average artery oxygen saturations of the diabetics is higher than the healthy people,which is significant difference statistically(P <0.01).The retinal each quadrant artery oxygen saturations of the diabetics are higher than the healthy people,however,only the superior temporal,inferior nasal and inferior temporal quadrants are significant difference statistically(P<0.01).The retinal average vein oxygen saturations of the diabetics are higher than the healthy people,but the difference isn’t statistical significance(P >0.05).The retinal superior nasal quadrant vein oxygen saturations of the diabetics are higher than the healthy people,which are significant difference statistically(P<0.01).The retinal arterio-venous oxygen saturations differences of the diabetics are higher than the healthy people,which is significant difference statistically(P <0.01).The retinal average artery and vein oxygen saturations of the syndrome of deficiency of both qi and yin are higher than other three syndromes,which is significant difference statistically(P<0.01).The retinal superior nasal quadrant and inferior nasal quadrant artery oxygen saturations of the syndrome of deficiency of both qi and yin are higher than other three syndromes,which is significant difference statistically(P<0.05).The retinal inferior nasal quadrant and inferior temporal quadrant vein oxygen saturations of the syndrome of yin and yang deficiency are lower than other three syndromes,which is significant difference statistically(P <0.01).Conclusion:1.According to the syndrome differentiation system,the type of TCM syndrome of type 2 diabetes mellitus patients is relatively objective,avoiding the blind supposition.2.The application of metabonomics technology to analyze the serum and urine differences between different syndrome types of metabolites that can make the diagnosis of TCM syndrome scientifically and quantitatively.3.The retinal average oxygen saturations of the diabetics are different with the healthy people,which shows that the retinal microcirculation changed in the condition of continuous hyperglycemia.It provides a new way for the prevention and treatment of diabetes and diabetic retinopathy.4.Among the different syndromes of traditional Chinese medicine of diabetics,the highest retinal average oxygen saturations is the syndrome of deficiency of both qi and yin,which confirms the rationality and scientificity of TCM syndrome differentiation of diabetes,which confirms deficiency of both qi and Yin is the key stage of diabetes development.5.Through the extraction and analysis of retinal information to study systemic diseases,from local tissue to whole,make up the deficiency of traditional Chinese medicine diagnosis by using the TCM eye diagnosis system,and promote innovation and development of TCM theory. |
