| To investigate the satisfaction of medical workers and patients in a county hospital of traditional Chinese medicine of Chongqing after comprehensive reform,a variety of methods,such as questionnaire and structured interview,were used to collect basic information about the county public hospital during 2012-2016,including human resources,infrastructure and financial situation(all 210 questionnaires designed for medical workers were issued;93 out of 100 questionnaires designed for patients were issued),followed by random personal interviews on medical reform with 20 hospital staff.On this basis,the data required was acquired.With those original materials further analyzed,the following preliminary conclusions could be reached.1.The effect of medical reform of the aforementioned county public hospital manifested in two respects after the comprehensive medical reform targeted to county public hospitals was implemented: one was the scores of medical staff ’ s satisfaction with medical reform lay between 69.0±16.0;the other was that administrativestaff were found more satisfactory with medical reform than medical staff.Relative compensation and assessment,medical staff showed lower satisfaction,with their scores lying between 9.0±4.0.2.Scores of patients’ satisfaction with the medical reform launched by the aforementioned county public hospital fell between 31.0 ± 8.0;satisfaction in municipal/county(district)hospitals was found significantly higher than that in the other institutions.3.According to pooled analysis of personal interviews with medical staff,interview results could be summed up as follows.(1)Balance of financial revenue and expenditure decreased year by year in the aforementioned county public hospital.(2)The hospital’s administration system was increasingly perfect.(3)Information construction was lagged behind in the hospital.(4)Popularization of traditional Chinese medicine theory-based therapies and services was fairly low.(5)It was lacking in talents of traditional Chinese medicine.As shown by the questionnaire result,problems came to light as public hospitals received medical reform,which could be generalized into the following four aspects.1.Financially,the lagged compensation mechanism,increasingly higher development cost after medical reform and the limitation of medical insurance policy led to unnecessary loss and excessively heavy financial burden.2.In terms of human resources,it was difficult for medical staff to apply for professional title appraisal for lack of high-end talents.3.The hospital fell short in traditional Chinese medicineservice utilization,talents in traditional Chinese medicine,power scientific research in traditional medicine and a sound pharmaceutical service system.4.Current payment mechanism of medical insurance that simply required patients to pay medical expense by disease category did more harm than good in facilitating steady development of the hospital.To address the problems above,the following policy suggestions are made.First and foremost,it is a top priority for the governments to strengthen financial investment and undertake their responsibilities.On the other hand,medical regulatory agencies should keep pace with government-led medical reform,promote scientific price reform of medical services and normalize hospitals.Secondly,it is also advisable to accelerate medical personnel training and induction,promote compensation and title reform steadily,perfect the payment of medical insurance and boost information reform systematically in order to give full play to the advantages of traditional Chinese medicine and quicken the implementation of comprehensive reform in hospitals of traditional Chinese medicine.In this study,the increasingly prominent problems that emerged in the comprehensive reform in a county public hospital is further analyzed and elaborated theoretically based on previous studies on the comprehensive reform of county public hospitals.Not only does this study contribute to the research of this field,but it also lays theoretical foundation for thesuccessful reform of Chongqing public hospitals of traditional Chinese medicine. |
