Objective:Evaluation of left ventricular systolic function in patients of endocardial fibroelastosis with leftventricular noncompaction(EFE&LVNC)using two-dimensional speckle tracking imaging.Methods:The subjects were divided into three groups(20 patients in each group),the EFE&LVNC group,the EFE group and the control group.The analystic parameters were longitudinal strain(LS),radial strain(RS)and circumferential strain(CS).These parameters were compared between two groups.Transmural gradient percentage(△TG%)was applicated to evaluate the rule of longitudinal strain level of each segment in three groups.Results:LS results:Compared with the EFE group,LS of basal posterior septum and the anterior segments in EFE&LVNC group were showed no difference.LS of basal lateral,the posterior segment,the anterior septum and the middle inferior segments were lower than those of EFE group(P<0.05).LS of other segments were significantly lower than that of EFE group(P<0.01);Compared with control group,the apical anterior segment in the EFE group was lower(P<0.05),while LS in other segments were significantly lower(P<0.01);Compared with control group,LS of all segments in EFE&LVNC group were significantly lower(P<0.01).The LS average of the same level segments results:compared with EFE group,LS in each level segment of EFE&LVNC group was significantly lower than that in EFE group(P<0.01);Compared with the control group,the level of LS in each level was significantly lower in EFE group than that in EFE group(P<0.01);Compared with the control group,the level of LS in each level of EFE&LVNC group was significantly lower than that in EFE group(P<0.01).EFE&LVNC group:apex<basal<middle;EFE group:basal<middle<apex;control group:basal<middle<apex.Comparison of △ TG% chart in three groups of basal-middle-apex level,the EFE&LVNC showed group reflex;EFE group showed bending angle;control group showed linear increase.RS results:RS between the EFE&LVNC group and the EFE group,RSof the basal anterior septum,the anterior,lateral,inferior segment and the posterior septum in the middle level and all apical segments were lower than that of the EFE group(P<0.05);Compared with the control group,RS of basal anterior septum,anterior,lateral,inferior segment,the middle anterior septum,anterior,posterior,inferior segment,posterior septum,and theapical lateral segment in the EFE group were significantly lower than those of the control group(P<0.01);Compared with the control group,RS of the basal anterior septum of EFE&LVNC was lower than that of the control group(P<0.05),RS of the other segments was significantly lower than that of the control group(P<0.01).CS results:Comparison between EFE&LVNC group and EFE group,CS of the middle anterior septum,anterior,posterior and inferior segment of the EFE&LVNC group were significantly lower than those of the EFE group(P<0.01),while the other segments were lower than the EFE group(P<0.05);Compared with the control group,CS of the basal posterior segment in the EFE group was significantly lower than that in the control group(P<0.01),CS of other segments was lower than that in the control group(P<0.05);Compared with the control group,CS of basal anterior septum in the EFE&LVNC group was lower than that of the control group(P<0.05),CS of the other segments was significantly lower than that of the control group(P<0.01).Conclusion:The left ventricular systolic function of EFE&LVNC patients was lower than that of EFE,which was the most serious in the apical segment,and was related to the distribution of the intraventricular trabecula.Transmural gradient percentage could be more sensitive to myocardial longitudinal gradient changes of multiple groups.
