| Objective: To discuss the value of Echocardiography for the diagnosis inthe noncompaction of ventricular myocardium and assess the left ventricularmyocardial systolic function in patients with noncompaction of ventricularmyocardium by using two-dimension Speckle Tracking imaging.Methods:26 patients with NVM were divided into two groups by left ventricular ejection fraction:NVM-1(LVEF<50%) and NVM-2(LVEF≥50%).and 50 healthy subjects as normal controls were included.Two-dimension images were acquired in all patients and healthy subjects.Then the heart chamber sizes, the ventricular wall thickness and left ventricular ejection fraction were measured. The blood flow of heart was showed by color doppler. Last,the peak systolic radial strain(RS)and longitudinal strain(LS) in NVM were calculated by 2-dimensional speckletracking imaging and compared among 3 groups such above.Results:1:All 26 patients’ involved myocardium mostly in the ventricular wall from middle segment to apex had diffused distribution of profuse reticular muscles and recessed fossae on the endocardial surface of myocardium. The color doppler showed there was low-speed blood ?ow within the fossae which was connected with the high-speed blood ?ow within the heart. 19 patients showed different degrees of heart failure.7 patients of cardiac function in normal,2 cases with other congenital heart disease.Compared with normal control group and NVM-2,LVDd was significantly increased,EF decreased in NVM-1,the differences was statistically significant(P < 0.05).The LVDd of NVM-2 was increased than the normal controls,the differences was statistically significant(P < 0.05);there was no statistically significant difference on the LVEF between the two groups(P>0.05). Compared with NVM-1,the number of affected segment for NVM-2 were less, the differences was statistically significant(P < 0.05);NC/C ratio difference between the two groups had no statistical significance(P>0.05). 2:The RS and LS of each level for NVM-1and NVM-2 were lower than the normal controls. the differences was statistically significant(P<0.05).The RS and LS of each level for NVM-1 were lower than NVM-2.There was significant difference between the two groups(P<0.05).Conclusion: 1:Echocardiography is not only clearly showing morphological characteristics of NVM, finding other cardiac abnormalities,butalso evaluating cardiac systolic function. It has an important value for NVM.2:The left ventricular myocardial systolic function could be detected by2D-STI sensitively.It has an important clinical value in early diagnosis and intervention. |
