Natural orifice translumenal endoscopic surgery(NOTES)is a technique that uses transvisceral access to perform surgical procedures entirely through a natural orifice, leaving no scars on the skin.At present, there are few reports about total transoral endoscopic throidectomy clinical experiences at home and abroad. Our hospital has already performed4cases,which will be discussed as followed about their clinical features.Current status of transoral endoscopic thyroid surgery will also be discussed.Objective:To analyze the clinical features of total transoral endoscopic throidectomy of4cases and discuss the current tatus of transoral endoscopic thyroid surgery.Methods:4cases of benign throid disease (confirmed by pathology after sugery) from November2013to April2014of Xiangya3rd Hospital were collected.We analyzed their clinical features,surgical indications, complications and so on. We also have literature study to discuss the current tatus of transoral endoscopic thyroid surgery.Results:4cases of total transoral endoscopic throidectomy in our hospital consisted of3female and1male patients.Average age is35years old. Average operation time is172min and postoperative residental time is6days.No major complication like horses,mental nerve palsy, bleeding or hematoma happened. No nursing complication happened.30-60days follow up showed complete satisfaction of the patients to cosmetic results of the surgery.Only one of the patients suffered from postoperative anterior cervical effusion, which was under controlled in a week. There are7reports and32cases all over the world on transoral endoscopic thyroid surgery till now. Besides several cases of recurrent laryngeal nerve palsy,there are no major operative complications occurred. Conclusion:1.There were no major postoperative complication happened except one case occurred anterior cervical effusion.The technique has remarkable cosmetic results and is proved to be safe and effective.2.Analysis to current clinical application of transoral endoscopic thyroid surgery shows that this surgery method is safe and effective,while still needs multiple-centered clinical control trial to be confirmed. |
