Objective:By observing the noninvasive positive pressure ventilation in combination with xingnaojing injection treatment of chronic obstructive pulmonary disease, the clinical curative effect of pulmonary encephalopathy, contrast before and after treatment the artery of patients with a Glasgow coma scale,pH,PaO2,PaCO2,neuron specific enolization enzyme changes, evaluation of noninvasive positive pressure ventilation in combination with xingnaojing injection in chronic obstructive pulmonary disease patients with pulmonary encephalopathy in the clinical application value,to combine traditional Chinese and western medicine in the treatment of pulmonary encephalopathy to offer effective theoretical foundation and treatment methods. Methods:A total of 60 patients with chronic obstructive pulmonary disease and pulmonary encephalopathy in ruikang hospital affiliated to guangxi university of traditional Chinese medicine from January 2016 to November 2017 were selected.by applying the method of random number table, patients were randomly divided into treatment group and control group30 cases each. Two groups of both conventional bronchodilator diastolic airway and methyl prednisolone anti-inflammatory,keeping water electrolyte imbalances,anti-infection treatment with the antimicrobial drug, pulmonary infection complicated with heart failure who sometime using improve cardiac function and other drugs.Control group in the conventional drug treatment at the same time give breathing machine ST noninvasive positive pressure ventilation mode.On the basis of control group and treatment group with xingnaojing injection 20 ml intravenous drip,1 times a day,two groups of a course of 5 days. Two groups before and after breathing difficulties, consciousness was observed and statistics in patients with tracheal intubation rate, were observed before and after treatment of patients with a Glasgow coma scale, arterial blood pH,PaO2,PaCO2,neuron specificity enolization enzyme changes. All 60 patients entered the result analysis,no case fall off. Results:Two groups of patients before treatment generally,Glasgou coma scale, arterial blood PH,PaO2,PaCO2,neuron specific enolization enzyme comparison, there were no statistically significant difference?P>0.05?.After treatment are:?1?the clinical curative effect comparison:treatment group total effectiveness 93.3%,control group total effective rate was66.7%.The total effective rate in treatment group is better than that of control group?P<0.05?.?2?:endotracheal intubation rate endotracheal intubation rate6.7%treatment group, observation group endotracheal intubation rate is26.7%,the treatment group endotracheal intubation rate is lower than the observation group.?3?Glasgou coma scale comparison:Glasgou coma score higher than the control group?P<0.05?,the treatment group was improved and brain function recovery is better than the control group.?4?the results of blood gas analysis comparison:PH,PaO2 and PaCO2 before and after treatment in the group comparison difference had statistical significance?P<0.05?,suggesting the intervention treatment, two groups can both improve hypoxemia and carbon dioxide retention.PH,PaO2,PaCO2 comparison between groups, the treatment group PH,PaO2,PaCO2 improvement than the control group significantly?P<0.05?,suggesting noninvasive mechanical ventilation economic cooperation with refreshing after intravenous injection intervention, more can improve hypoxemia and carbon dioxide retention.Specificity?5?neuron-specific enolase comparison:blood group after treatment, two groups of neurons specificity enolization enzyme before and after treatment were decreased?P<0.05?,a comparison between groups, the treatment group blood neuron specificity enolization enzyme content than the control group decreased significantly?P<0.05?.Two groups of patients with no serious adverse events occurred in the process of treatment. Conclusion:Noninvasive positive pressure ventilation in combination with refreshing static injection treatment of chronic obstructive pulmonary disease, pulmonary encephalopathy curative effect significantly, can promote the disturbance of consciousness recovery faster, and improve the respiratory function, reduce the rate of tracheal intubation,on clinical has good practical value.
