| Objective : To investigate the serum Angptl2 levels in type 2 diabetes mellitus(T2DM)patients and to explore the relationship between Angptl2 and carotid atherosclerosis(CAS)in patients with T2 DM.And to explore the mechanism of Angptl2 in type 2 diabetes mellitus and carotid atherosclerosis by analyzing the characteristics and influencing factors of serum Angptl2.Methods: 114 Patients with T2 DM were divided into two groups according to the presence of carotid plaques: group T2DM+CAS(58 cases with carotid plague)and group T2DM+NAS(56 cases without carotid plague).56 healthy individuals were served as controls(group NC).Age,sex and course were recorded.Height,weight,blood pressure,total cholesterol(TC),triglycerides(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),high sensitivity C-reactive protein(hs-CRP),fasting plasma glucose(FPG),fasting serum insulin(FINS),glycosylated hemoglobin(HbA1c),were determined in all subjects.Body mass index(BMI)and homeostasis model assessment for insulin resistance index(HOMA-IR)were calculated.ELISA method was used to detect the level of serum Angptl2.Results:(1)Serum Angptl2 levels were significantly higher in T2 DM + CAS group and T2DM+NAS group(69.83±12.07,42.93±10.44)ng/mL than in NC group(24.26±8.78)ng/mL(P < 0.01).T2DM+CAS group showed significantly higher serum levels of Angptl2 compared to that of T2 DM + NAS group(P < 0.01).(2)Pearson correlation analysis showed that serum Angptl2 was positively correlated with age、BMI、SBP、DBP、TC、TG、LDL-C、FPG、HbA1c in T2 DM patients(r=0.574,0.325,0.528,0.308,0.396,0.387,0.295,0.536,0.601;P<0.01),and negatively correlated with HDL-C,FINS in T2 DM patients(r=-0.248,-0.352;P < 0.01).(3)Multiple regression analysis showed that FINS 、 HbA1 c were the independent factors of the serum Angptl2 levels(β=-2.186,2.680;P<0.01).(4)Logistic regression analysis showed that serum Angptl2 levels were one of the independent risk factors of carotid artery atherosclerosis.Conclusion :(1)Serum Angptl2 levels were significantly higer in T2DM+NAS group and T2DM+CAS group than in NC group.Angptl2 was closely correlated with BMI,FPG,HbA1 c and FINS.This result indicates that Angptl2 may be associated with the occurrence and development of type 2 diabetes mellitus and Angptl2 may play a facilitated role in the occurrence and development of type 2 diabetes mellitus.(2)Compared with the NC group,the serum Angptl2 levels in T2DM+NAS group were increased,which were further increased in T2DM+CAS.Angptl2 was closely correlated with age,TC,TG,LDL-C,HDL-C,and Angptl2 was the independent factor of CAS,suggesting that the increase of serum Angptl2 levels may be involved in the development of atherosclerosis in T2 DM patients.Angptl2 may play an important role in the development and progression of carotid atherosclerosis in T2 DM patients,suggesting that serum Angptl2 levels may be an early indicator of carotid atherosclerosis in T2 DM patients.(3)Serum Angptl2 was positively correlated with age,BMI,SBP,DBP,TC,TG,LDL-C,FPG and HbA1 c,and negatively correlated with HDL-C and FINS,FINS and HbA1 c were the independent factors of the serum Angptl2 levels,suggesting that Angptl2 levels are closely related with age,obesity,blood pressure,blood glucose and blood lipids,Angptl2 is possible to participate in the development and progression of type 2 diabetes and carotid atherosclerosis through the above factors. |
