Objectives:Recently,Plasma testosterone concentration plays an important role in the development of type 2 diabetes(T2DM)and vascular complications.More and more scholars pay attention to this phenomenon. This study intends to determine the level of T and CIMT in male T2 DM population,to analyze the relationship between the two,to explore the role of T in AS and to discuss its mechanism,to provide a theoretical basis for early detection and prevention of AS.Methods:The T2 DM inpatients were selected from November 2016 to January 2017 in the department of Endocrinology and Metabolism in the Second Affiliated Hospital of Nanchang University.A total of 154 patients were enrolled in the study.154 cases of male patients with T2 DM according to whether the CIMT thickening is divided into CIMT thickening group,CIMT normal group.Patients were overnight fasting for 8hours to determinate Serological markers and To measure everyone’s blood pressure, height, weight, waist and Computing BMI,HOMA-IR(CP).Carotid artery ultrasound was used to measure CIMT and PM detection rate. The measured values were analyzed by using SPSS21.0.Results:1.Comparison of the general data between the two groupsThe TT and FT concentrations in patients with CIMT thickening group were lower than those in normal group(P<0.01).There was an difference between the two groups. The Hs-CRP,IL-6,HbA1 c,HOMA-IR(CP),waist,age,and Patch ratio of the thickening group was higher than that of the normal group(P<0.05 or P<0.01).There was no difference between the two groups in TG,TC,HDL-C,LDL-C,FBG,BMI,BP,smoking history, hypertension and family history of diabetes.2.Correlation between markers and CIMTTT and FT were negatively correlated with the thickness of CIMT,and inflammatory factors(Hs-CRP,IL-6),HbA1 c,systolic blood pressure, waist circumference, age and CIMT thickening was positively correlated.3.The correlation between serum testosterone level and metabolic indexesThe average TT of all subjects was 11.28nmol/l.The average FT of all subjects was0.42nmol/l.Compared with Tertile1 group,T2 group and T3 group CIMT thickening cases, inflammatory factors(Hs-CRP,IL-6)were decreased, and there was significant difference(P < 0.05 or P < 0.01).With lower testosterone levels,it showed a negative linear correlation with Hs-CRP,IL-6 and CIMT.With the TT value from high to low FT also decreased linearly, suggesting that the TT and FT changes are synchronized in this study population.4.Risk factors for T2 DM combined with CIMT thickeningThe CIMT as the dependent variable, adjusting for age,BMI,HbA1 c,FBG,TT,FT,IL6,Hs-CRP,multivariate Logistic regression analysis showed that risk factors of T2 DM and CIMT are IL6,Hs-CRP thickness,OR,and values were 2.014,2.011;protection factor are TT,FT,OR,and values were 0.266,0.289..Conclusions:1.T2 DM was negatively correlated with CIMT in middle-aged and elderly male patients, and the decrease of testosterone was a risk factor for AS.2.Plasma testosterone was negatively correlated with CRP and IL-6 and plasma testosterone may be associated with CIMT thickening by mediating inflammatory responses. |
