ObjectiveExplore the relationship between anxiety,insomnia and gastrointestinal function,by observing different anxiety degree of the clinical characteristics of insomnia patients with abnormal defecate,in order to guide clinical diagnosis and treatment.Formulate the basic TCM therapy according to the TCM syndrome characteristics and clinical experience,and conduct a clinical curative effect observation study,so as to provide some reference to the TCM therapy for insomnia patients with abnormal defecate.MethodA study was conducted on 71 cases of insomnia accompanied by defecation of patients in the second area of brain disease of Dongzhimen Hospital of Beijing University of Traditional Chinese Medicine from July 2017 to January 2018.Understand the patients’ general information,work way of life,habits,personality,etc.,by using the TCM four diagnostic information questionnaire to(the)understanding of patients with symptoms of TCM pulse and tongue condition,using the Pittsburgh sleep index scale(PSQI),Hamilton anxiety scale(HAMA),depression self rating scale(SDS)were respectively to evaluate patients’ sleep quality,anxiety,depression,core yi with HXD-1 tester for patients with heart rate variability and brain activity test,blood test interleukin-6 and high-sensitivity c-reactive protein levels,and TCM intervention therapy in patients with insomnia associated with abnormal defecate,with liver spleen and treatment as the core,with four inverse scattering and definite pill and subtract operation on the basis of party,combining with the patient’s specific symptoms and signs for syndrome differentiation treatment,2 weeks after treatment on patients with Pittsburgh sleep index scale(PSQI),Hamilton anxiety scale(HAMA)evaluation again,evaluation of sleep and anxiety of the patients before and after treatment was observed,established according to the collected data,a relational database using SPSS 20.0 statistical analysis software to descriptive statistics,nonparametric test and chi-square test data analysis,correlation analysis,etc.ResultsDivided 71 insomnia patients with abnormal defecate into anxiety(HAMA score>14)group and non anxiety(HAMA score 14 points or less),which the total number of anxiety group 27 cases(38.0%),the anxiety group total 44 cases(62.0%).There were 13 cases(18.3%)in the anxiety group with mild defecation,and 4 cases(5.6%)in the patients with moderate defecation,and 5 cases(7.0%)in the patients with severedefecation.In the non-anxiety group,there were 26 cases(36.6%),17 cases(23.9%),and 1 case(1.4%)of patients with severe defecation.The mean of HAMA score of patients with severe defecation was higher than that of patients with mild and moderate defecation,and the difference was statistically significant(P<0.05).The anxiety group’s rank mean of PSQI score sleep quality,sleep time,sleep time,sleep efficiency,sleep medication and daytime dysfunction score are greater than the non-anxiety group.The HAMA score of insomnia patients with abnormal defecation was positively correlated with PSQI score(P<0.05).The anxiety group’s rank mean of SDS score was higher than that of the non-anxiety group,and the difference was statistically significant(P<0.05).The HAMA score of insomnia patients with abnormal defecation was positively correlated with SDS score(P<0.05).There was no significant correlation between the HRV index and the brain function index(P<0.05)in the anxiety group and the non-anxiety group.The anxiety group’s rank mean of hsCRP level was higher than that of the non-anxiety group,the difference was statistically significant(P<0.05).The HAMA score and hsCRP levels of insomnia patients with abnormal defecate were positively correlated(P<0.05),no obvious correlation with interleukin 6 levels(P>0.05).The PSQI score and HAMA score were both reduced after 2 weeks’ TCM intervention for 30 insomnia patients with abnormal defecate,and the difference was statistically significant(P<0.05).The improvement rate of insomnia after treatment was 80%.Conclusions1 There was a certain correlation between the degree of anxiety and the degree of abnormal defecation in the patients with insomnia,and the higher the degree of anxiety,the greater the degree of abnormal defecation.2 The degree of anxiety was positively correlated with the degree of insomnia and depression,and the level of anxiety was positively correlated with the level of hs-CRP.The higher the anxiety level,the worse the sleep quality,the higher the depression level,and the higher the inflammatory level.3 The sleep quality and the anxiety levels both improved and had certain curative effect after the treatment of the dispersing stagnated liver qi and strengthening spleen.
