| Purpose:To compare the Traditional Chinese Medicine(TCM)syndrome and sign and related indexes in 45 cases of patients with early diabetic nephropathy and in 40 cases of patients with type 2 diabetes(ACR<30mg/g),and to summarize and analyze the clinical and TCM syndromic characteristics in 45 cases of patients with early diabetic nephropathy,and carry out a Meta analysis of the therapeutic effect of nvigorating qi,nourishing yin and activating blood in the treatment of early diabetic nephropathy.in order to provide an effective evidence for the clinical prevention and treatment of early diabetic nephropathy.Material and method:In article 1,we selected 85 patients diagnosed with type 2 diabetes from department of endocrinology in the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from October 2015 to October 2016.There are 45 cases with early diabetic nephropathy among them.We made statistical analysis of differences in general condition,blood pressure,BMI,WC,retinopathy,some laboratory indexes,TCM syndromes and sign,and clinical symptoms between the group with early diabetic nephropathy and control group(ACR<30mg/g),as well as differences in related indexes between two groups of the same TCM syndrome and sign of Qi-Yin deficiency and blood stasis type.the distribution of syndrome,main symptoms,tongue and pulse condition in TCM,and cardiovascular complication of early diabetes kidney,differences of relevant indexes between obesity group and normal group.In article 2 we retrieved RCT articles published in medical journals from January 2006 to October 2016,in which TCM treatment of nvigorating qi,nourishing yin and activating blood was used as intervention,and ACEI treatment was applied to the experimental group and the control group,The literature included was analyzed with Rev.Man5.3 software.Results: 1.Between early diabetic nephropathy and control group,there is a statistical difference in the course of the disease(P<0.05);There are also differences in laboratory indexes such as glycosylated hemoglobin,low density lipoprotein,serum creatinine and uric acid(P<0.05).There is a significant difference in the diastolic blood pressure(P<0.05);The most common symptoms in the two groups are fatigue,frequent urination,thirst,pain and numbness of limbs,blurring of vision;The most common type of syndrome is Qi-Yin deficiency and blood stasis;Between the two groups of the same type of syndrome: Qi-Yin deficiency and blood stasis,there are significant differences in glycosylated hemoglobin and serum creatinine(P<0.05).2.The most common symptoms of diabetic nephropathy group are fatigue,frequent urination,thirst,pain and numbness of limbs,blurring of vision.The common tongue color is dark red or dark.Tongue coating is white and lack of body fluid,Thready deep pulse is the main type of pulse.The most common type of syndrome is Qi-Yin deficiency and blood stasis.distribution of syndrome : Qi-Yin deficiency and blood stasis type(22/49%)>Yin deficiency and blood stasis type(12/27%)> Qi deficiency of spleen and kidney and blood stasis type(7/16%)> Yin-Yang deficiency and blood stasis type(4/8%).The level of triglyceride and cholesterol is statistically different between Obesity group and normal group(P<0.05).3.Meta analysis: The control group used ACEI drugs in the treatment of early diabetic kidney disease,The experimental group use Yiqi Yangyin Huoxue drugs based on the control group,there were significant differences in UAER,BUN,Hb A1 C,FPG,LDL-C and TC between the experimental group and the control group(P<0.05).Conclusion: 1.Qi-yin deficiency and blood stasis is the main syndrome of both diabetes and early diabetic nephropathy.2.Course of disease,Hb A1 C,blood lipids,blood pressure,uric acid are closely related to the early diabetic nephropathy.3.The treatment of nvigorating qi,nourishing yin and activating blood combined with ACEI can effectively improve renal function,blood glucose and blood lipid. |
