Innovation Expert Advice Law Developed Early Diabetic Nephropathy Questionnaire And The Result Analysis

Objective:Using modified Delphi method, making 《Based on virtual blood stasis pathogenesis essence law of TCM diagnosis and treatment of early diabetic nephropathy study》 expert questionnaire to carry out methodological innovation and exploration. Through the method of innovation and exploration of the next step, trying to provide methodology reference for revealing syndrome rules of early diabetic nephropathy, which can also provide theoretical basis for the research of early diabetic nephropathy syndrome characteristics to some extent.Material and method:In this study, by consulting related literature concerning early DN of Chinese medicine, integrated traditional and Western medicine in modern medical journals and books during past10years, to summarize the occurrence frequency of early diabetic nephropathy symptoms, lesions, syndrome differentiation, treatment principles and the required physical and chemical of inspection indicators which are reported in the literature, and to create the entry pool. Design expert questionnaire using the Delphi method and drawing on the method of production scale. Filter entries by using frequency method to form the main part of the questionnaire, and apply the validity and reliability tests to check the reliability and validity of the expert questionnaire. Complete the first round of expert questionnaire in this way, then send it to experts. After recovery of expert questionnaire of the first round, the group members will make a statistical analysis according to the results, then the opinions of these experts will be integrated in order to develop the second round of expert questionnaire. SPSS17.0Statistical Package are used for the establishment of a database and statistical analysis during both round results of expert questionnaire. Pay great attention to the concentration ratio and the coordination degree of these advices, positive coefficient of experts and reliability index of expert questionnaire. Then distinguish and sort out these entries, to reveal the syndrome characteristics of the early diabetic nephropathy initially.Results:1. Two rounds of experts investigation were conducted in this research, both expert positive factors were100%, which indicates that experts paid great attention to this study, the credibility of the research results was higher.2. The reliability of both rounds experts questionnaire were between0.7-0.98, which indicates that internal consistency of the description questionnaire was very high, and the experts questionnaire was designed reasonably.3. According to the statistical analysis of the second round of experts questionnaire, the symptoms characteristic of early diabetic nephropathy was revealed initially:The representative symptoms of early diabetic nephropathy are described as follows:nocturia, frequent urination and more, malaise and fatigue, edema, thirst hi drink, numbness, weak waist, shortness of breath, disinclination to talk, sleepy head and body weight, waist and back pain, dysphoria fever, irritability, insomnia, hands and feet, chills, sweating, numbness, palpitations; lesions of early diabetic nephropathy:kidney, network, spleen; Syndrome types of early diabetic nephropathy are as follows:qi and Yin Deficiency, spleen and Kidney qi and Yin Deficiency, kidney Yang Deficiency loss, blood stasis, kidney yin deficiency, yang deficiency of spleen and kidney (gas), kidney deficiency; Treatment principles of early diabetic nephropathy are as follows:notifying kidney and spleen, notifying qi and yin, notifying qi and promoting blood circulation, promoting blood circulation, notifying kidney and spleen, promoting blood circulation, kidney Qi meridians, warming kidney and spleen qi and nourishing yin and promoting blood circulation, kidneyphysical and chemical examination the Gujing; Early diagnosis of diabetic nephropathy are required as follows:urine albumin, glycated hemoglobin,24h urinary albumin excretion rate, fasting blood glucose,2h postprandial blood glucose, urine, serum creatinine, urinary albumin/creatinineurea nitrogen, blood lipids, serum albumin, ion (K+, Na+, Cl-, of Ca2+and Mg2+, P3,+), liver function.Conclusion:We followed the evidence-based medicine spirit, took Delphi method into reference which is commonly used in management science and scientific prediction science. Then we made 《Based on virtual blood stasis pathogenesis essence law of TCM diagnosis and treatment of early diabetic nephropathy study》 expert questionnaire through drawing on the research methods of scale, combining the characteristics of Chinese Medicine, carrying out some aspects of research such as literature investigation of early DN, the collection of expert experience and so on. Through the statistical analysis of both rounds experts questionnaire’results, the figure was that Cronbach’s alpha coefficient was between0.70-0.98, which belongs to the high reliability and illustrate that the application of expert opinion survey method of early diabetic nephropathy syndrome factors is feasible. The making method of experts questionnaire provide methodology reference for the next step that revealing early diabetic nephropathy syndrome rules, which also provide positive reference and guidance function for Chinese medicine clinical and scientific research of early DN.
