Objective: To treat facial moderate to severe acne,we developed an optical fiber imported intra-tissue photodynamic therapy: the optical fiber irradiation 5-aminolevulinic acid photodynamic therapy(OFI-ALA-PDT).The aim of this study was to compare the treatment effect and tolerability of OFI-ALA-PDT versus traditional ALA-PDT in the treatment of facial moderate to severe acne vulgaris.Methods: 60 patients with facial moderate to severe acne were enrolled into the OFI-ALA-PDT group and the traditional ALA-PDT group,with 30 patients in each group.The different between these2 groups was the red light irradiation methods used.In the OFI-ALA-PDT we used intra-tissue irradiation(import the red light directly into the target lesion with optical fiber)for 5 min,while the traditional ALA-PDT group received whole-face irradiation for 20 min.All patients avoided light packet for 1-1.5h before every treatment and received 1 irradiation every 7 to 10 days for a total of 6 irradiation.Treatment effects and adverse reactions was recorded after the4 th and 6th irradiation,at 4,8,16 weeks after the entire treatment.SPSS17.0software was used for statistical analysis,P<0.05 was considented as statistically significant.Results: 1.Clinical efficacy:⑴After the 4th irradiation,the effective rate in the OFI-ALA-PDT group(90.0%)was higher than that in the traditional ALA-PDT group(66.7%),P<0.05.⑵After the 6th irradiation,the effective rate in the OFI-ALA-PDT group(93.3%)was higher than that in the traditional ALA-PDT group(90%),but P> 0.05.⑶At 4 weeks after the entire treatment,the effective rate in the OFI-ALA-PDT group(93.3%)washigher than that in the traditional ALA-PDT group(86.7%),but P> 0.05.⑷At8 weeks after the entire treatment,the effective rate in the OFI-ALA-PDT group(90.0%)was higher than that in the traditional ALA-PDT group(86.7%),but P> 0.05.⑸At 16 weeks after the entire treatment,the effective rate in the OFI-ALA-PDT group(86.7%)was higher than that in the traditional ALA-PDT group(83.3%),but P> 0.05.2.Adverse events: these adverse events include itching,edematous erythema,pain,pustules,blisters,pigmentation,reaction acne,desquamation,etc.⑴There were 182 adverse events in the OFI-ALA-PDT group and 497 in the traditional ALA-PDT group,which showed a significant different(P<0.05).⑵The adverse event rate in the OFI-ALA-PDT group(53.3%)was lower than that in the traditional ALA-PDT group(83.3%),which showed a significant different(P<0.05).Conclusion: 1.There was no significant difference between these groups after the 6th irradiation and at 4,8,16 weeks after the entire treatment,these results indicated that two kinds of therapies had a similar efficancy.2.By the end of4 th irradiation,the effective rate in the OFI-ALA-PDT group was significantly higher than that in the traditional ALA-PDT group,these results showed that OFI-ALA-PDT had improved effective rate in the early stage of irradiation.3.The adverse events in the OFI-ALA-PDT was fewer than that in the traditional ALA-PDT,these results showed OFI-ALA-PDT treatment performded better in decreasing the reactions.
