Computer-aided Taylor Spatial Frame In The Treatment Of Severe Comminuted Tibia And Fibular Fractures

Objective:(1)To retrospectively study the curative effect and complications of computer-aided Taylor Spatial Frame(TSF)in the treatment of severe and complicated tibiofibular fractures,and provide a theoretical basis for the clinical application of this technique.(2)To compare the efficacy and complications of TSF and internal fixation in the treatment of severely complicated tibia fractures.Methods:(1)From May 2013 to June 2017,28 patients of severe complicated tibiofibular fractures treated with TSF were treated in our department,including 23 males and 5 females;17 open fractures(Gustilo Ⅱ is 12 and Ⅲ5),11 patients of closed fractures(8 cases of grade 2 and 3 cases of grade 3).The clinical data of 28 patients were followed up and the therapeutic effects and complications of TSF for treating severe and complicated tibiofibular fractures were evaluated.(2)The clinical efficacy and complications of 42 cases of severe tibiofibular fractures treated with internal fixation in our department from January 2011 to March 2017 were compared.Conclusion:(1)The computer-assisted Taylor three-dimensional space external fixator has a definite curative effect in the treatment of severely complicated tibial fractures,and has a low incidence of complications.It can achieve the advantages of fracture closure,precise and rapid reduction,and provides a clinical treatment of complex tibial fractures.A new way.(2)Compared with the internal fixation group,the TSF group spent less time than the internal fixation group in terms of preoperative preparation time,operation time,fracture healing time,complete weight loading time,hospitalization days,etc.,and hospitalization costs were lower,the difference was statistically significant.(P<0.05);Compared with postoperative complications in TSF group and internal fixation group,the incidence of postoperative infection and osteomyelitis was lower in the TSF group than in the internal fixation group(P<0.05);There was no significant difference in the incidence of healing,nonunion and re-fracture between the two groups(P>0.05).
