Safety Of Ovarian Preservation In Premenopausal Women With Early Endometrial Cancer:A Meta-analysis

Objective:The aim of this article was to investigate the safety of preserving ovary in premenopausal women with endometrial cancer(EC).Methods:Online databases,including PubMed,EMbase,The Cochrane Library,Web of Science,CNKI,CBM,WanFang Data and VIP were searched from inception to 3rd January 2018,to collect all cohort studies or case-control studies on the safety and prognosis of preserving ovary in premenopausal women with endometrial cancer.At the same time we manually search related domestic journals and magazines.Two reviewers independently screened literature,extracted data and assessed the risk of bias of included studies according to the inclusion and exclusion criteria.Then,we used RevMan 5.3 software to analyze data.Results:A total of 18 retrospective cohort studies involving 30,490 patients were included,and 3,246 patients of ovarian preservation(OP).The results of meta-analysis showed that,for OP and bilateral salpingo-oopherectomy(BSO),there was significant difference in mortality of 10years after surgery[OR=0.62,95%CI(0.49,0.78),P<0.0001],Postoperative quality of life[MD=7.79,95%CI(0.58,14.99),P=0.03],Postoperative E2 level[MD=115.66,95%CI(103.89,127.43),P<0.00001],Postoperative LH level[MD=-51.94,95%CI(-90.18,-13.70),P=0.008]and Postoperative FSH level[MD=-44.16,95%CI(-47.02,-41.30),P<0.00001].However,these two groups,there was no significant difference in postoperative recurrence rate[OR=1.30,95%CI(0.70,2.40),P=0.41],mortality of 5 years after surgery[OR=0.75,95%CI(0.53,1.05),P=0.10].Conclusion:Meta-analysis showed that preserving ovary in premenopausal women with early endometrial cancer(EC)is safety,and is good for the Postoperative quality of life.
