PurposesTo discuss the significance of TAP test in cancer diagnosis and cancer screening.Evaluate the TAP test in cancer detection,thus to further clarify the value of the TAP test in the tumor screening in the population,in order to guide the reasonable application of the TAP test in cancer screening by comparing the TAP expression level among the tumor group,precancerous lesion group,and normal/benign disease group.Method:Collect 4998 patients in the physical examination center,outpatient and inpatient department who required for tumor screening from October 2016 to October 2017 in Sir Run Run Shaw Hospital affiliated to Zhejiang University.All the participants were carried out TAP test.Collect their gender,age,TAP expression level,BMI,smoking and drinking history,family history of tumor,pathological results,corresponding imaging examination,etc.The eligibles are divided into cancer group,precancerous lesion group and normal/benign disease group.Compare the TAP numerical value and among the tumor group,precancerous lesion group,normal/benign disease group.Contrast the expression level of TAP level and TAP positive rate among different groups,P<0.05 was statistically significant.Calculate the TAP detection sensitivity,specificity,positive predictive value,negative predictive value,Youden index and the area under the receiver operating characteristic curves.Finally,Logistic regression was used to analyze the correlation between tumor occurrence and TAP numerical value,age,gender,smoking,alcohol consumption,family history of tumor and BMI.And the tumor prediction model was established.Result;In this study,46 tumor patients were screened,among which 15 patients were lung cancer(4 patients with early lung cancer),including 8 patients with TAP positive and 7 patients with TAP negative.There were 11 cases of gastric cancer,among which 8 cases were TAP positive.There were 5 cases of liver cancer,6 cases of colorectal cancer,4 cases of breast cancer and 5 cases of other tumors,all of which were TAP positive.The proportion of gender,age,TAP numerical value,TAP positive rate,BMI,smoking and drinking history,family history of tumor among the cancer group,precancerous lesion group and normal/benign disease group have differences,P<0.05,the differences were statistically significant.The TAP positive rate of cancer group,precancerous lesion group and normal/benign disease group were 78.3%,3.1%,1.0%,P<0.001,the differences were statistically significant.The TAP positive rate of tumor group was significantly higher than the TAP positive rate of precancerous lesions,P<0.001,the differences were statistically significant.The TAP positive rate of precancerous lesions was significantly higher than the normal/benign disease group,P<0.001,the differences were statistically significant.The positive rate and precancerous lesion group is higher than normal/benign disease group,P<0.001,the difference was statistically significant.The TAP numerical value of tumor group was 156.902(112.791)μm2,which was significantly higher than the TAP numerical value of the precancerous lesion group56.675(35.129)μm2 and the TAP value of the normal/benign disease group 55.954(24.768)μm2,P<0.001,the difference was statistically significant.The TAP numerical value of the precancerous lesion group was 56.675(35.129)μ2,which was higher than that of the normal/benign condition group 55.954(24.768)μm2,and P>0.05,the interaction between the two was not significant.The positive predictive value of TAP was 36.7%,and the negative predictive value was 99.8%.The accuracy of TAP test was 98.6%.The sensitivity of TAP test was 78.3%.The Youden Index was 77.0%.The area under the receiver operating characteristic curves was 0.905,95%confidence interval(0,835-0.975),P<0.001,the difference was statistically significant.Logistic regression analysis results suggests that tumor was related with TAP expression level,age,smoking,drinking and family history of cancer.The exact OR were as follow respectively:1.054,1.054,2.313,2.353,2.313,P values<0.05,the difference was statistically significant.Tumor prediction probability value regression model Y = 1/(1+ EXP(0.054*TAP value + 1.886 X-10.516)],the area under the ROC curve of tumor prediction probability value regression model is 0.925,95%confidence interal(0.865-0.985),P<0.001,there are significant difference statistically significant.Conclusion:TAP test has high sensitivity and accuracy for tumor screening and it is a kind of simple,broad spectrum,efficient tumor diagnosis and screening method.It can be carried out to screening for tumors in population and provide the reference for the diagnosis of tumors.
