| Objective: To study the change of expression of microRNA-126 and microRNA-155 before and after treatment of muddy phlegm Syndrome in patients with coronary heart disease,and to provide some theoretical basis for the molecular biology of Tian Xiangdan treatment for thoracic and turbid phlegm.Methods: 60 patients were evenly divided into basic treatment + Tianxiangdan group,western medicine basic treatment group and control group.The patient’s syndrome was investigated using TCM symptom score table,and the relevant risk factors、clinical laboratory efficacy and safety indicators,cervical blood vessels Color Doppler ultrasonography and coronary angiography results were analyzed,and microRNA-126 and microRNA-155 were extracted using Real-time PCR quantitative method to detect changes in plasma expression,and were compared with each other.Results: 1.Tian Xiangdan has the effect of improving the symptoms of coronary heart disease,and compared with Western medicine treatment group,cholesterol,low density lipoprotein significantly reduced,with significant differences;2.Correlation between cervical vascular ultrasound,coronary angiography and microRNA-155: lesion counts and IMT(r=0.970,P=0.000),TPA(r=0.947,P=0.000),Course scores(r=0.45,P=0.000),Gensini score(r=0.945,P=0.000)all had a positive correlation;IMT had no significant correlation with microRNA-155(r=-0.395,P=0.333)in one-vessel disease.However,there was a significant correlation between microRNA-155(r=-0.953,P=0.000)in double-vessel disease and three-vessel disease;IMT was positively correlated with Gensini score(r=0.972,P=0.000);Gensini score and microRNA-155(r =-0.907,P =0.001)was negatively correlated,both statistically significant;3.The expression of microRNA-126 and 155 was increased in microRNAs before and after treatment of muddy phlegm Syndrome of coronary heart disease compared with healthy group: the relative expression of microRNA-126 in the control group was 0.96(0.91~1.01),relativeto microRNA-155.The expression level was 1.01(0.86~1.16);the relative expression levels of microRNA-126 before and after treatment in the basic treatment group were 1.00(0.79~1.21)、1.10(0.99~1.21),respectively;the relative expression of microRNA-155 was 0.91(0.71 ~ 1.11).)、 1.00(0.79 ~ 1.21);Tianxiangdan group before and after treatment relative expression of microRNA-126 0.72(0.61~0.82)、1.07(0.93~1.20);relative expression of microRNA-155 0.59(0.41~1.54)、1.17(1.03~1.30).Compared with the control group,the basic treatment group microRNA-155(Z=-4.25,P=0.0001)was significantly upregulated,the difference was statistically significant;the basic treatment group microRNA-126(Z=-1.14,P=0.26),the difference was not statistical Significance;Compared with the control group,Tianxiangdan group microRNA-126(Z=-4.14,P=0.001)and microRNA-155(Z=-4.78,P=0.0001)were significantly upregulated,and the difference was statistically significant;and according to ROC The curve shows that the area under the microRNA-155 curve(AUC)is 0.394(95% CI:0.766~0.965,P=0.2334),the sensitivity is 47.3%,the specificity is 46.4%,and the area under the microRNA-126 curve(AUC)is 0.786(95% CI: 0.796 ~ 0.876,P=0.001),sensitivity 72.5%,specificity 85%.Conclusion: 1.Tianxiangdan can improve the symptoms of coronary heart disease.Compared with Western medicine treatment group,cholesterol and low-density lipoprotein are significantly reduced,with significant differences;2.The number of lesions was positively correlated with IMT,TPA,Course scores,and Gensini scores,and IMT was positively correlated with coronary angiography Gensini score.Neck color Doppler ultrasound can be used as a basis for the diagnosis of coronary heart disease to some extent;3.The degree of coronary artery lesion was negatively correlated with the expression level of microRNA-155,and according to the ROC curve,microRNA-126 had a certain role in the diagnosis of coronary heart disease,while microRNA-155 had no diagnostic significance;4.Tianxiangdan can significantly up-regulate the expression of microRNA-126 and microRNA-155 in plasma,inhibit lipid metabolism and inflammatory reaction to a certain extent,and inhibit the development of atherosclerosis. |
