Analysis Of Disease Economic Burden For Patients With End-stage Hepatic Alveolar Echinococcosis Undergone Autologous Liver Transplantation

Objective: Good clinical outcomes have been achieved in treating hepatic alveolar echinococcosis(HAE)using autologous liver transplantation(ALT)technique in a hospital in Xinjiang Province.Since there almost no any relative work studying the disease burden of HAE patients,so this study was aiming to investigate the disease burden and its impact factors for patients with HAE who underwent ALT.Besides,quality of life(QOL)tests were applied in order to better estimation and further investigation of HAE patients.Method: Basic clinical and all kinds of relative costs lists of 51 HAE patients that accepted ALT surgery during August,2010 to February,2017 were obtained from hospital clinical records.Both direct and indirect economic burden(calculated using Disability Adjusted Life Year,DALY method)was calculated and analyzed using Logistic stepwise regression method.SF-36 questioner was applied to assess the QOL of HAE patients.Results: Total disease economic burden was 447128.89 RMB.Direct economic burden was 257943.45 RMB that accounts for about 57.54%,and among this the direct medical cost was 257292.09 RMB,when non-medical direct cost was 651.36 RMB.Indirect disease economic burden was 189834.06 RMB that accounted for approximately 42.46%.The average hospitalization days,occupation and age are the significant influencing factors of direct economic burden of HAE patients.QOL score showed the lowest level at 1 month after operation,and then gradually increased until reaching the normal levels at 3 month after operation.All of the scales were almost same comparing with general people at post-op 6 month.Conclusion: DALY is an essential parameter in estimating indirect disease economic burden.Disease burden of HAE was very heavy,and it is urging to make more proper medical aid strategies according to the different levels of hospitals.It is reasonable for reducing economic burden for HAE patients that enhancing of education and screening should be tightened and conducting more individual therapeutic methods are essential.Multi-central,large scaled and high qualified studies are required for further analysis.
