| Objective:this study through the primary gout patients(observation group)and the healthy physical examination(control group)infrared thermal image inspection,liver,spleen,kidney calorific value of the data,statistical analysis and discussion of primary gout TCM syndrome type and the relationship between the infrared thermal image,provide the basis for the syndrome differentiation of primary gout.Methods:collected in December 2016-December 2017 between visits in the third affiliated hospital of yunnan institute of rheumatology outpatient service(kunming hospital of traditional Chinese medicine)of 100 patients with primary gout and the same period 100 cases of healthy check-up,application of Beijing yue days photoelectric technology company produces the HIR-2001 medical infrared thermal imager,acquisition of liver,spleen,kidney and limb joints,calorific value,establishing database of excel,using SPSS21.0 statistical software analysis.Result:1.Observation group syndrome distribution13 cases(13%)of damp-heat aggregating type;20 cases(20%)of stasis,38 cases(38%)of phlegm block,29 cases of liver and kidney Yin deficiency(29%).Sputum block type(38%)> liver and kidney Yin deficiency type(29%)> stasis heat block type(20%)> humid heat accumulation type(13%).2.The observation group was compared with the control group of liver,spleen and kidney The infrared thermal imaging values of the left side and right side of the observation group were both higher than the control group(both P < 0.01).The large abdominal infrared thermal image value was lower than the control group(both P < 0.01).Both the left waist and the right waist infrared thermal image value were lower than the control group(both P < 0.01).3.The comparison of infrared thermal images of liver,spleen and kidney in the observation group The large abdominal infrared thermal imaging value of the sputum blocked type was lower than that of stasis and liver and kidney Yin deficiency(both P < 0.01).The infrared thermal image values of the left waist and right waist of liver and kidney Yin deficiency type were higher than those of damp-heat accumulation,stasis,and phlegm block type(all P < 0.01).4.Analysis of infrared thermal image of extremities joints The joint distribution of the lesions in the observation group: 52 cases with plantar toe joint > 46 cases of the ankle > 13 cases with knee joint > 1 case of shoulder joint and1 case of elbow joint.The infrared thermal image value of the lateral joints of the patients with moist heat accumulation and stasis was higher than that of the healthy side.The thermal image value of the joint infrared thermal image was higher than that of the healthy side.The infrared thermal image value of phlegm blocking type lesion was lower than that of the healthy side.There was no significant change in the joint and the healthy side of liver and kidney Yin deficiency.Conclusion:1.In patients with primary gout,liver depression,spleen deficiency,kidney deficiency,and thermal infrared heat map are widely changed.Infrared thermal imaging detection can provide a basis for the development of primary gout prevention and treatment.2.The symptoms of spleen deficiency in patients with phlegm blocking gout are particularly obvious,and the clinical treatment should be taken from the aspects of invigorating spleen and dehumidification.The deficiency of kidney Yin deficiency in patients with liver and kidney Yin deficiency is more prominent. |
