Study On The Basic Drug Accessibility Of Community Health Service Institutions After The Implementation Of The Basic Drug System

Objective: to study the district community health service system for basic drugs after implementation of essential medicines procurement,distribution,use and price changes,and so on and so forth,evaluation of essential medicines in community health service institutions of availability and affordability conditions,in order to improve the accessibility of basic drugs,improve national essential drug system to provide the reference basis.Methods: This study through literature to understand the basic drug system implementation present situation at home and abroad and accessibility.Using existing data platform to collect the 7 community health service centers and 44 community health service station of essential medicines procurement,distribution,use and price data.Prescriptions,at the same time,the sampling investigation,a total of 4985 copies of prescriptions.In addition,this study also interview drug purchase of medical institutions,community doctors and the relevant person in charge of distribution companies and 20 people,analysis the influence factors of basic drug accessibility.This research use SPSS17.0 on statistical analysis of data collected by using the method of descriptive statistics study after the introduction of the basic drug system drug purchase,distribution and usage,using t test and chi-square test methods compared before and after the implementation of basic drugs affordable changes.Results: statistical area community health service institutions to implement the basic drug system three years,the proportion of basic drug procurement achieves purchase drugs more than 95% of the total,and presents the trend of increasing year by year.Procurement of essential medicines varieties increased from 535 to 732.Shortage declines year by year the trend in the process of purchasing drugs,these drugs reduced from 17 kinds of 10 kinds of total.Three days delivery rate is low,basic drugs 7 delivery rate is higher,but failed to achieve the requirement of the delivery rate of 100%.Community health service institutions use of basic drugs amount to 96.24%,and a rising trend year by year,but the proportion of national essential drug use is low,only accounts for 20.8% of the total sum,single prescription number 0.74,using the national essential drug use amount of top 20 drugs,accounted for only 30% of national essential drugs.For drug price comparison before and after the basic drug system,20 kinds of commonly used drug prices fall on average by 30.86%,the biggest drop to 56%.8 kinds of common standard therapy to treat disease using the standard of the average per capita disposable income of drug costs the equivalent of a multiple of from 0.27 times to 0.12 times,is equal to the per capita consumption expenditure,multiples by 0.43 times to 0.18 times the standard.Clinic visits growth was 27.79%,the per capita outpatient drug expenses decreased from 59.28 yuan to 47.31 yuan,a drop of 20.21%.Conclusion: Community health service institutions after the implementation of the basic drug system,the basic drug purchasing channel unobstructed,purchasing medicines preferred essential medicines,purchase quantity and purchase amount increased year by year,the shortage of its quantity reduced year by year.Clinical medicine is given priority to with essential medicines,using amount increased year by year.System,the basic drug prices,8 kinds of common standard therapy to treat disease medication cost is equal to the per capita disposable income and consumption expenditure per capita,standard multiples,community outpatient pharmaceutical costs are significantly lower per capita,the basic drug availability and affordability improved significantly.Because is at the early stage of system implementation,and there also exist the following problems: shortage of cheap drugs,people recognized brands limited use of drugs,basic drugs 3 distribution rate is low,the national essential drug varieties use ratio is too low,the masses of the basic drugs awareness is not strong and so on,hindered the promotion of basic drugs.Consolidate and improve the system for basic drugs and improve the accessibility of basic drugs work a long way to go.
