| Objective: Health is one of the minimum survival needs of human beings and the basis of other rights.Medical care has become a new focus of China’s health services in the 21st century.In recent years,the Chinese government has been committed to the reform and efforts in the direction of equalization of medical services.Among them,the national essential drug system is an important step to achieve the equalization of basic medical services,which is attracting increasing attention.Methods: This paper compares the evaluation and analysis index system of the implementation effect of various essential drug systems through literature research.The final decision is to select the evaluation system of outcome indicators in the operational package for assessing,monitoring and evaluating country pharmaceutical situations issued by WHO,which is the most widely used at present.Through questionnaires and interviews,nearly 100 pharmacists,doctors,managers and patients from 9 community health service centers in Shanghai were investigated.The availability of 13 medicines in WHO essential medicines core catalogue and common pediatric medicines,the price and affordability of commonly used essential medicines in Shanghai,the rational use of drugs and the quality of medicines were investigated.This paper evaluates the effect of the implementation of the essential drug system in Shanghai in the past 10 years,and analyses the effect of the implementation of the essential drug policy in Shanghai and the problems that need to be further solved.Results: Applying the evaluation index to evaluate the implementation of Shanghai’s essential drug system,it was found that Shanghai’s community health service centers had good homogeneity,almost no regional differences,and most of the medicines available were very good.However,paediatric drugs are seriously lacking and some antibiotics and antidepressants are not available;Drug quality,preservation and disposal conditions are very good,management is perfect;drug price is almost a price in the whole city,fairness is better;Drug affordability is generally better after medical insurance reimbursement,but in the long-term use of multiple drugs,or at their own expense,affordability is very poor;Lack of uniform standard treatment guidelines.Through this study,we can judge the achievements and shortcomings of the implementation of the essential drug system;The results can be compared horizontally between different regions,and can also be compared vertically in the same region,which can provide a scientific basis for the continuous improvement of the system.Conclusion: It is found that Shanghai Community Health Service Center is not equipped with enough pediatric drugs and antidepressants,which is not conducive to the promotion of the graded diagnosis and treatment system.The affordability of essential medicines is poor in the case of long-term multi-drug use and without medical insurance.We need to focus on solving these problems in order to further promote the steady progress of equity in basic medical and health services. |
