Objectives:This survey is to understand the current situation of the usage of opioids in patients with cancer pain in a Grade ?A hospital in Changchun,identify the possible problems and provide targeted education for the use of opioids;and to provide evidence for further improving patient compliance and efficacy of using opioids,effectively reducing cancer pain,and improving the quality of life in patients with cancer pain.Methods:A total of 74 patients who were admitted in a Grade ?A hospital in Changchun from July 2016 to June 2017 were selected as the subjects,and the selection of patients was limited by having taken opioid drugs at home and having not received treatment in hospital before.Questionnaires were performed at the time of admission,at 1st week and 1st month after treatment of cancer pain.The questionnaire includes the general situation,knowledge on using opioid,attitude on using opioid,behavior on using opioid,and pain situation.After the questionnaire was collected,the data were input into Epidata3.2 using double blind.And the data were statistically analyzed by SPSS18.0 software.Results:1.The total score of the questionnaire is 156 points,including 100 points for knowledge part,50 points for attitude part,and 6 points for behavior part.For the initial use of opioids,the total score of 74 patients was 74.96 ± 13.79,including 47.62 ± 9.13 for knowledge,25.15 ± 5.30 for attitude,and 1.99 ± 1.98 for behavior.Therefore,the level of knowledge is low,the patient's attitude towards the use of opioids is not recognized,and medication behavior is poor for the initial use of opioids.2.The average score for knowledge on using opioid was 47.62,67.26 and 85.70 points in the initial use,one week after medication and one month after medication.The difference between the three groups was statistically significant(F = 318.569,p <0.001).The average score for attitude on using opioid was 25.25,32.73 and 42.32 in the initial use,one week after medication and one month after medication.The difference between the three groups was statistically significant(F=266.571,p<0.001)?The average score for behavior on using opioid was 1.99?4.38 and 5.61 in the initial use,one week after medication and one month after medication.The difference between the three groups was statistically significant(F=144.502,p<0.001).3.There were significant correlations between the knowledge,attitude and behavior of opioid usage using the Pearson correlation analysis.There is a significant correlation between the knowledge and attitudes of opioid usage(r = 0.827,p <0.001),between knowledge and behavior of opioid usage(r = 0.758,p <0.001),and between attitudes and behavior of opioid usage(r = 0.718,p <0.001).4.The patients with moderate to severe pain were 55(74.3%)at the initial use of opioid,37(50%)after one week of medication,and 7(9.5%)after one month of medication.With the increased treatment time of cancer pain patients,the knowledge on opioid has grown,the attitude of opioid usage has been corrected,and the medication behavior has been improved.So,control effect on cancer pain has been improved.Conclusion:1.The knowledge score and attitude score were in the middle levels and the medication behavior was poor in the cancer pain patients at the initial use of opioids.These indicated that the insufficient understanding of opioid related knowledge and too anxious on the possible adverse reactions of opioid lead to the poor patient compliance and poor pain control.2.The scores of knowledge,attitudes,and behavior of cancer pain patients were gradually improved with continuous education on opioids use during treatment.Therefore,it is necessary for patients with cancer pain to receive opioid knowledge education to improve the cognitive level of patients and help patients better cope with cancer pain treatment.3.There is a correlation between the knowledge,attitude and behavior of opioid use.So,it suggests that it is useful to conduct a survey to understand the lack of knowledge and misconceptions on opioid,which could provide targeted education for cancer pain patients.These educations could increase the beliefs on opioid usage,promote medication compliance,so as to improve the cancer pain control effect and to improve the quality of life of patients.
