| Objective In the study of chronic obstructive pulmonary disease(COPD)analysis of the occurrence status of patients and depression and related factors of depression in patients with COPD were obtained to study the related evaluation index in clinical application value,provide the basis for nursing and treatment for the emotional state of patients with COPD in order to improve the quality of life of patients.Methods Choose between September 2015 and September 2015,tianjin medical university general hospital gao gan families,ward,respiratory medicine ward adult(older than 50 years old)of 60 patients with COPD,the diagnosis should be accord with the Chinese medical association in 2013 breathing neurology,chronic obstructive pulmonary disease committee of guide to the diagnosis and treatment of chronic obstructive pulmonary disease.After admission made unified according to the criteria for the cases of single observation in recording data,the basis of clinical need and use in the form of questionnaire to the self-assessment and his review of the clinical symptoms in patients with a combination of data acquisition and application of Hamilton depression rating scale(HAMD)survey of patients with depressive symptoms and the clinical cases of collected data to the corresponding statistical analysis of data,get the incidence of chronic obstructive pulmonary disease in this region;Analysis of copd combined with depression and smoking index,BMI,and lung function index correlation;Get depressed patients with copd pulmonary non-specific symptoms such as fatigue,difficulty in breathing index,and their own health and daily life quality of the relevance of the risk factors and evaluation of the value of the questionnaire;Application SPSS 17.0 statistical software to statistical analysis,mean ±standard deviation be used for data results;Group of measurement data comparison between the independent sample t-test analysis;The relationship between HAMD depression score and clinical questionnaire scores using bivariate correlation Spearman correlation coefficient method and reported Spearman correlation coefficient r value;Using binary logistic regression screening of COPD with independent impact factors of depression.With P < 0.05 for the difference was statistically significant.Results1.In 60 cases of patients with COPD 31 cases of men,women,29 cases.The sex ratio of 1.07 :1,aged 58-89,average age(73.45 + 6.93),all the 60 patients with lung function,MMRC questionnaire,HAMD questionnaire,concise fatigue scale BFI questionnaire,existed results 21 cases of patients with depression(HAMD score 7 or higher),the survey of COPD associated with the incidence of depression was 35%,COPD associated with depression,10 cases were male,female COPD associated with depression in 11 cases,male to female ratio of 1:1.1.2.Body mass index(BMI),smoking index SI,chronic obstructive pulmonary disease process is the influence factors of COPD associated with depression,P values(0.021,0.018 respectively,< 0.001)difference was statistically significant.SI smoking index(P value for OR value of 1.005,0.021,95% CI1.001 ~ 1.009),chronic obstructive pulmonary disease process(OR P value for value of 1.534,0.001,95% CI 1.188 ~1.982)were independent factors affecting COPD associated with depression.3.Degree of depression patients with copd CAT scores,difficulty breathing MMRC score,fatigue index BFI score between a linear positive correlation(r = 0.837,0.653,0.842,p < 0.001),the difference was statistically significant.Associated with lung function FEV1 expected no significant(r = 0.139,P value is 0.291)no statistically significant difference.Conclusion1.The incidence of chronic obstructive pulmonary disease with depression was higher in Tianjin.2.The severity of chronic obstructive pulmonary disease(COPD)combined with depression was correlated with BMI,smoking,and course of disease.3.The CAT score,the degree of fatigue and shortness of breath of patients with COPD were correlated with depression,and the relationship between FEV1 score and lung function was not significant.4.The fatigue index BFI provides an effective method for evaluating COPD combined with depression. |
