[Objective]To explore the relationship between related factors and chronic obstructive pulmonary disease(COPD)combined with anxiety and depression and their influence on the level of anxiety and depression,to understand the common TCM syndromes of COPD combined with anxiety and depression,so as to provide certain theoretical basis for future diagnosis and treatment.[Methods]In this study,inpatients and outpatients with COPD admitted to the Respiratory Department of Xinhua Hospital of Zhejiang Province from January 2021 to March 2022 were enrolled in a cross-sectional survey.The Hospital Anxiety And Depression Scale(HADS)was used for evaluation.According to the results of HADS-A and HADS-D,the COPD patients were divided into COPD group,COPD with anxiety group and COPD with depression group,and the anxiety and depression degree of the patients were graded.Statistical analysis of the distribution differences between the groups in age,gender,smoking history,corticosteroids,course of disease,and mMRC score.Logistic regression was used to analyze the possible influencing factors of COPD with anxiety and depression,and to investigate the distribution of TCM syndrome types of COPD with anxiety and depression.[Results]A total of 107 COPD inpatients and outpatients admitted to the Respiratory Department of Xinhua Hospital of Zhejiang Province from January 2021 to March 2022 were included in this study,including 61 patients with simple COPD,19 patients with anxiety,36 patients with depression,and 9 patients with anxiety and depression.The detection rate of depression was about twice that of anxiety.There were differences in the distribution of age,course of disease,Body Mass Index(BMI),ICS,COPD grade and mMRC score between the COPD group with depression and the non-depression group,but there were no differences in all factors between the groups with different degrees of depression.The distribution of mMRC score was different between the anxiety group and the non-anxiety group,and there was no difference in all factors between the different anxiety groups.Logistic regression analysis showed that age,BMI,ICS,mMRC and COPD grade were the influencing factors of depression in COPD patients.There was statistical significance between the distribution of TCM syndromes and anxiety and depression.In COPD group,deficiency of lung and spleen and deficiency of lung and qi were the dominant syndromes,while in anxiety and depression group,deficiency of lung and kidney qi and deficiency of lung and kidney qi and Yin were the dominant syndromes.[Conclusion]The incidence of anxiety and depression is relatively higher in COPD patients.With the increase of age,the prolongation of disease course,the use of ICS,the decrease of BMI,the increase of COPD grade and mMRC score,the likelihood of depression increased;In addition,mMRC score was also associated with the occurrence of anxiety.TCM syndrome type is related to anxiety and depression,but may not be an important factor.In the treatment of patients with COPD combined with anxiety and depression,attention should be paid to the treatment of psychological problems.
