Objective : By investigating the present status and relevant factors of cognitive impairment in high-risk crowds of stroke living in community of Xining,to understand the incidence of cognitive impairment in high-risk population of stroke in middle altitude area and its influencing factors,by analyzing the total score and the each score of Mo CA in population with cognitive impairment,to understand the field vulnerable to damage and the degree of damage in high-risk crowds of stroke,we want to provide a reference for control the relevant risk factors and treat cognitive impairment at early stages,improve the quality of life for crowds who having high-risk of stroke.Methods :By screening of population with high-risk of stroke who aged over 40 years in Xiguan Street,Xining City,258 patients with high-risk of stroke were selected.We use Mo CA to evaluate their cognitive condition and they were divided into 2 groups:cognitive impairment group with 208 cases in it;normal group with 50 cases in it.We measure blood glucose,blood lipids and carotid ultrasonography,collect all the data.Analyze the relationship between the relevant factors and cognitive impairment.Results:In this study,we investigated a total of 258 people with high-risk of stroke in Xining community,including 208 cases of cognitive impairment group,accounting for 80.6%,50 cases of normal group,accounting for 19.4%.Compared with normal group,the proportion of age,education level,stroke history and carotid plaque was significantly higher in the cognitive impairment group and the difference had statistical significance(P<0.05).Logistic analysis showed that the history of stroke and the age over 60 years old were independent risk factors of cognitive impairment in stroke crowds with high risk.OR(95% CI)were 0.085(0.011~0.651),0.113(1.336~6.546).The scores of Mo CA with 3,4,5 and over 6 risk factors were(27.74 ±1.73),(27.22 ± 1.66),(26.33 ± 1.25)and(24.33 ± 1.26).The scores of Mo CA decreased with the increase of the number of risk factors,the difference had statistical significance(F = 49.851,P <0.05),and compared with each other the difference had statistical significance(P <0.05).The total scores of cognitive impairment group were lower than normal group,the difference was statistical significance(P <0.05).The scores of cognitive impairment group in visual space and executive function,attention,language,abstraction and delayed recall were lower than normal group,the difference had statistical significance(P <0.05).There was no significant difference between the two groups in terms of naming and orientation(P>0.05).The scores of previous stroke group were lower than normal group,the difference had statistical significance(P<0.05).The scores of previous stroke group in visual space and executive function,attention,language,abstraction and delayed recall were lower than normal group,the difference had statistical significance(P<0.05).Conclusion:1?The incidence of cognitive impairment in high-risk population of stroke in Xining community was higher,along with the increase in the number of risk factors,the cognitive impairment is more severe,mainly in terms of space and execution,attention,language,abstract,delayed memories have damage,and the naming and orientation have no significant damage.2?Patients with a history of stroke have a greater risk of cognitive impairment than those without a history of stroke,and their cognitive impaired significantly in all fields.3?Age,low education level,history of stroke,carotid plaque can increase the risk of cognitive impairment in high-risk population of stroke,age,history of stroke is the independent risk factors of cognitive impairment in high-risk population of stroke.
