| Objective : 1.Try compiling the postnatal depression questionnaire for Chinese maternal and child health care staff and evaluate it.2.Investigate the postnatal depression cognitive status of maternal and child health care staff in Shihezi city and analyze the influencing factors.Methods:1.Literature method:arrange and analyze the related literature of maternal and child health personnel on postpartum depression cognition,based on the theory of cognitive and K-A-P theory mode,reference to the study of postpartum depression,discuss the postnatal depression cognitive content of maternal and child health staff.2.Expert consultation method:18 experts were selected using Delphi method,carried out two rounds of expert consultation.3.Consult the relevant experts,on-the-spot investigation,establish the questionnaire items pool on base of them and compile the initial questionnaire.Use the convenient sampling method,make a pre-survey by selecting a small sample of 46 cases.We combine with the survey results,the views of respondents,the advice of experts and recommendations,apply related statistical analysis method,group discussion results,we amend the questionnaire and make a formal new one.4.Questionnaire method:to facilitate the survey of 231 maternal and child health care staffs and adopt reliability and validity analysis,correlation analysis and other statistical methods to evaluate the questionnaire;use the sampling method,investigate 355 maternal and child health care staffs in Shihezi city with self-designed questionnaire.5.Interview method:a random sample of 16 maternal and child health care staffs to have face-to-face semi-structured interviews.6.Statistical analysis method:we use the SPSS statistical software to analyze the questionnaire,adopt statistical method of the descriptive statistical analysis,t test,Pearson correlation analysis,variance analysis and multiple stepwise regression analysis according to the nature and purpose of research data,using Colaizzi’s 7 step analysis to analyze the interview results.Results: 1.The use of network database in university library,access to 17 of China’s maternal and child health care staff to study cognitive postnatal depression literature,the number of the imbalance between regions,the eastern developed regions the total is relatively more,less research in central and western regions,only 8 papers introduced the use of assessment tools to study.2.18 experts were invited to use the Delphi method for expert consultation,including 13 in the field of obstetrics and gynecology,2 in the community,2 in psychology and 1 in epidemiology and statistics.The effective questionnaires were recovered by 100% two rounds of consultation,the average value of expert authority coefficient was 0.84,the expert opinions were more concentrated and the coordination coefficients of two rounds of consultation were 0.416 and 0.330 respectively.3.The postnatal depression questionnaire for maternal and child health care staff include:the basic situation registration form of maternal and child health staff,the postnatal depression cognitive questionnaire of maternal and child health staff,the cognitive questionnaire consists of 100 items and 4 dimensions,dimension 1:the understanding and learning situation of postnatal depression concept,dimension 2: the cognition of postnatal depression influencing factors,dimension 3: the cognition of postnatal depression clinical manifestations and hazards,dimension 4:the cognition of evaluation,diagnosis and treatment of postnatal depression.4.Cronbach’s a coefficient of postnatal depression questionnaire for maternal and child health care staff is 0.917,the split half reliability coefficient is 0.936,the test-retest correlation coefficient is 0.956(P <0.001),scale-level content validity index(S-CVI)is 0.942,the criterion validity correlation coefficient is 0.865(P <0.001).5.355 maternal and child health care staffs are from 19 to 60(33.94±9.013)years old,the working time is from 1 to 40(11.05±8.799)years,and the total postnatal depression score of respondents is(76.01±10.332),the dimension 1 score is(9.85 ±1.918),the dimension 2 score is(38.67 ±5.656),the dimension 3 score is(17.10 ±2.829),the dimension 4 score is(10.38± 4.771).6.The single factor analysis showed that age,educational level,marital status,post,working time,department,title affected cognitive status score of postnatal depression of maternal and child health staff(P<0.05).Stepwise regression analysis showed that age,educational level,department,marital status,post affected cognitive status score of postnatal depression of maternal and child health staff(P<0.05).7.The interviews results show that the maternal and child health care staff did not master the relevant knowledge or skills of postnatal depression in its entirety,the cognition of evaluation,diagnosis and treatment of postnatal depression is poor,they lacked follow-up management for postnatal depression,the key to the future work of postnatal depression is preventing and health care.Conclusion: 1.Maternal and child health care staff in our country lack of awareness of postnatal depression.2.In this study,the compiled cognitive questionnaire of postnatal depression of maternal and child health care staff had a clear structure,comprehensive content and was representative with good reliability and validity,which accorded with the requirement of questionnaire compilation and could be used to evaluate the cognitive status of postnatal depression of maternal and child health care staff.3.The overall cognitive status on postnatal depression of maternal and child health staff in Shihezi city was in the middle level,and they need further enhance the cognition,the assessment and diagnosis cognition of maternal and child health staff to postnatal depression were poor.4.Age,educational level,marital status,post,working time,department,title affected cognitive status of postnatal depression of maternal and child health staff.5.Age,educational level,department,marital status and post of maternal and child health staff had influences on postnatal depression cognitive status of maternal and child health care staff,they had a close relationship with postnatal depression cognitive status. |
