Objectives1.Conclusion early postoperative complications of congenital cataract surgery of the overall situation.2.To study a new method of postoperative evaluation for patients with congenital cataract.3.To study the application value of UBM and the far infrared fundus reflex camera based on principle of Off-oxis in the evaluation of postoperative complications in patients with congenital cataract.4.To assess the diagnostic criteria of UBM and the far infrared fundus reflex camera based on principle of Off-oxis on the postoperative complications of congenital cataract.Methods1.To study the infant congenital cataract patients during April 2004-2016 December in Guangzhou women and children's medical center by retrospective analysis,403 cases of congenital cataract patients(713 eyes),238 cases in male,165 cases in female.To analyze the overall situation of the occurrence of postoperative complications of congenital cataract.Intraocular lens implantation time was divided into two groups.Group A:patients aged less than one year of intraocular lens implantation,93 cases,138 eyes;group B: patients aged less than two years of implantation of intraocular lens,297 cases,with a total of 521 eyes.The surgical procedures were as follows: extracapsular cataract extraction and anterior vitrectomy + intraocular lens implantation.The postoperative complications of the two groups were compared and analyzed by X2 test.In addition,the division of children into groups by age,the C group underwent surgery: congenital cataract in less than 2 years old with congenital cataract extraction with anterior vitreous body resection or extracapsular cataract extraction with anterior vitreous body +intraocular lens implantation in patients of 254 cases with 449 eyes;group D: 2 years old of congenital cataract congenital extracapsular cataract extraction with anterior vitreous body resection or extracapsular cataract extraction with anterior vitreous body +intraocular lens implantation in patients of 147 cases with 238 eyes,and compared two groups of patients' postoperative complications,and the data were analyzed by X2 test.2.To summarize the image features of patients with congenital cataract and the complications in the use of UBM.Sensitivity and specificity were used to assess the diagnostic value of UBM in congenital cataract surgery.Observed the 146 cases patients with congenital cataract(235 eyes)during in September 2012 to December 2016 in Guangzhou women and children medical center by surgical microscope or slit lamp microscope,and summarize the types and image features of postoperative complications of congenital cataract by UBM.The diagnostic value of UBM in postoperative complications of congenital cataract was evaluated by sensitivity and specificity.3.To sum up the image feature of patients accepted far infrared fundus camera technology reflecting which based on the principle of Off-oxis inspection after congenital cataract surgery.To evaluate the diagnostic value of far infrared fundus imaging technique based on Off-oxis principle in the postoperative complications of congenital cataract by sensitivity and specificity.To exam the 132 cases follow-up outpatients(218eyes)after congenital cataract surgery during March 2016 to March 2017 in in Guangzhou women and children's Medical Center by far infrared reflective fundus camera technology based on Off-oxis principle,slit lamp,UBM or surgical microscope.To summarize the image features of patients with congenital cataract after far infrared ray fundus reflex imaging based on Off-oxis principle.To assess the sensitivity and specificity of Off-oxis far infrared fundus camera technologybased on Off-oxis principle and UBM method for visual inspection after congenital cataract surgery,then analyze the effectiveness and practicability of the two different inspection methods.Results1.In this study,the rate of surgery for congenital cataract patients was 76% before 2years old,of which 3-6 months were the most.A small number of patients with congenital cataract implanted intraocular lens before 2 years old,while most patients with congenital cataract after implantation of artificial lens after the age of 2.2.The complications of postoperative cataract,iris adhesion,strabismus,secondary glaucoma,intraocular lens position abnormality after congenital cataract patients before 2years old who treated with the first and second IOL implantation respectively,have no statistically significant,no difference between the two groups in postoperative complications.3.The cases of surgery for patients with congenital cataract postoperative complications within 2 years old were higher than >2 years old after surgery for congenital cataract patients.The probability of postoperative cataract,iris adhesion in two group after cataract surgery have statistically significant.4.Analyze the effectiveness of the complications after congenital cataract surgery,the results show that the sensitivity and specificity of UBM to diagnose the complications of postoperative cataract,secondary glaucoma,iris adhesion,abnormal position of intraocular lens was 100%.In the practical analysis,the positive predictive value of UBM for the diagnosis of postoperative complications of congenital cataract was 96.2%,the negative predictive value was 100%,96.8%,the rate of missed diagnosis by UBM was 0,and the false positive rate was 1.1%.5.The sensitivity of far infrared fundus camera technology based on the principle of Off-oxis method for visual transparency diagnosis was 100%,and specificity was 97.4%.The positive predictive value of far infrared fundus camera technology based on the principle of Off-oxis method and UBM for visual transparency diagnosis was 85.3%,and93.5%;the negative predictive value was 100% and 100%,the rate of missed diagnosis was 0,and the false positive rate was 2.6% and 1.04%,respectively.Conclusions1.surgery in younger age,the risk of these two complications are higher.The probability of strabismus,nystagmus in two group after cataract surgery have statistically significant.The probability is related to age,younger age with lower complications.The probability of secondary glaucoma,intraocular lens position in two groups have no statistically significant,they have no obvious relationship with the age of surgery.2.Ultrasound biomicroscopy microscope(UBM)is a safe inspection tool with high diagnostic value to diagnose congenital cataract surgery complications: posterior cataract,synechia,corneal edema and abnormal intraocular lens placement3.The far infrared reflective fundus camera technology based on Off-oxis principle to screen congenital cataract and has a high value to diagnose congenital cataract surgery after Los opacitas,which is a safe,simple and applicable inspection technology.4.According to the size,shape and thickness of the pupil area crystal residue by UBM inspection,location with intraocular lens and iris,pupil area crystal residue can be divided into three types: membranous cataract,cortical regeneration type,mixed type.5.Ultrasound biomicroscopy(UBM)examination combined with the far infrared fundus reflex camera technology based on principle of Off-oxis method can can effectively assess the outcome of congenital cataract after cataract surgery.
