Relationships Between Sex Hormones And Blood Lipid And Visceral Fat Area In Postmenopausal Women With Type 2 Diabetes Mellitus

Objective:1.To analyze the characteristics of sex hormones in postmenopausal women with type 2 diabetes mellitus(T2DM);2.To analyze the characteristics of blood lipid and visceral fat area in postmenopausal women with T2DM;3.To explore the relationships between sex hormones,blood lipid and visceral fat area in postmenopausal women with T2 DM.Methods:All the subjects were collected from March 1,2015 to December 31,2016 at endocrinology department in Shandong Provincial Hospital.147 subjects with T2 DM were collected among postmenopausal women.Among them,26 patients with menopause time 1-5 years were defined as group A;30 patients with menopause time 6-10 years were defined as group B;91 patients with menopause time more than 10 years were defined as group C with 91.The patients’ information were collected and registered including 1,general data(hospital number,age,duration of diabetes,past history,menstruation,blood pressure,BMI and etc);2,biochemical indexes(fasting plasma glucose,blood lipid,sex hormones,liver function,kidney function and etc);3,visceral fat area;4,subcutaneous fat area.The three groups of patients were analyzed retrospectively.The relationships among sex hormone,blood lipids and visceral fat area were estimated by one-way ANOVA analyses and multiple linear regression analyses.Results: 1.Comparisons of each index of research subject among the three groups With the increase of menopausal time,coronary heart disease and cerebrovascular disease of the three groups were gradually increased and there was no significant difference among them(P=0.080,0.081);With the increase of menopausal time the prevalence of fatty liver decreased gradually and there was no significant difference(P=0.483).With the increase of menopause time,the age,systolic blood pressure and creatinine of the three groups were increased significantly(P=0.000,0.021,0.004),alanine aminotransferase decreased significantly(P=0.007)and total cholesterol increased first and then decreased(P=0.045).There was no significant difference among duration of diabetes,diastolic blood pressure,height,weight,BMI,follicle stimulating hormone,luteinizing hormone,estradiol,testosterone,high density lipoprotein cholesterol,low density lipoprotein cholesterol,triglyceride,visceral fat area,subcutaneous fat area,fasting plasma glucose,glycosylated hemoglobin,fasting C peptide,insulin resistance index,aspartate aminotransferase,gamma glutamyl transferase,alkaline phosphatase and uric acid of the three groups(P>0.05).2.Multiple linear regression analyses among the three groups After adjustment for confusing factors,total cholesterol was positively related to fasting plasma glucose(β=0.246,P=0.002)and follicle stimulating hormone(β=0.173,P=0.032).Total cholesterol was positively related to fasting plasma glucose(β=0.254,P=0.001)and luteinizing hormone(β=0.238,P=0.003).There is no relationship between sex hormone and high density lipoprotein cholesterol.Low density lipoprotein cholesterol was positively related to fasting plasma glucose(β=0.225,P=0.006)and follicle stimulating hormone(β=0.169,P=0.037).Low density lipoprotein cholesterol was positively related to fasting plasma glucose(β=0.234,P=0.004)and luteinizing hormone(β=0.222,P=0.006).There is no relationship between sex hormone and triglyceride.Visceral fat area was positively related to BMI(β=0.695,P=0.000),testosterone(β=0.189,P=0.009)and HOMA-IR(β=0.166,P=0.018).There is no relationship between sex hormone and subcutaneous fat area.3.Comparasions between two groups of subjects divided using testosterone median The low density lipoprotein cholesterol and visceral fat area in high testosterone group was higher than that in low testosterone group significantly(P=0.045,0.001);High density lipoprotein cholesterol in high testosterone was lower than that in low testosterone group,and there was no significant difference(P=0.440);Total cholesterol,triglyceride and subcutaneous fat area in high testosterone was higher than that in low testosterone group and there was no significant difference(P=0.108,0.169,0.055).Conclusion:Research results showed that: with the increase of postmenopauseal time with type 2 diabetes,total cholesterol increased first and then decreased.Total cholesterol and low density lipoprotein cholesterol increased with the increase of follicle stimulating hormone and luteinizing hormone.Visceral fat area increased with the increase of testosterone.The low density lipoprotein cholesterol and visceral fat area of high testosterone group were significantly higher than that of low testosterone group.
