Objective:To develop and validate an individualized precisely navigation template using reverse engineering,obverse engineering and three-dimensional computer simulation program,which can be used in knee arthroplasty,and test its preliminary clinical application research.Methods:20 cases of patients undergoing total knee arthroplasty,were randomly divided into two groups,two groups of 10 cases,3 D printing plate osteotomy guide group preoperative pelvic double spiral C T scan of the lowest point of the ankle,the patients with skeletal reconstruction of computer model software,analysis and calculation of cutting and cutting angle bone osteotomy direction,so as to design a navigation template,surgical planning and Simulation of operation.According to intraoperative osteotomy and inserting a knee prosthesis in advance through the digital technology design of Department of orthopedics osteotomy angle.The traditional mechanical positioning operation group with an intramedullary positioning method.The accuracy of postoperative C T scanning and three-dimensional reconstruction analysis and compared between the two groups.Statistical 2 weeks after operation HSS knee score,knee joint activity the accuracy of analysis,compared with two group of surgery limb alignment angle index.Results:3 D printing osteotomy guide plate group 2 weeks after operation HSS knee score was(87 ±2.7),knee joint activity of(102.3 ±3.3)degrees,lower limb force line angle(the angle between the femoral and tibial mechanical axis was(2.0 ±0.4)degrees.The traditional mechanical positioning operation group 2 weeks after operation HSS knee score(86.3± 2.6)degrees,knee joint activity(101.6 ± 4.2)degrees,the lower extremity angle(the angle between the femoral and tibial mechanical axis)(2.5± 0.5)degrees.3 D print osteotomy guide plate group and the traditional mechanical positioning operation group patients 2 weeks after operation HSS knee score,knee joint activity showed no significant difference(P> 0.05),and there was statistical significance between the lower extremity angle difference(P<0.05).Conclusion:3 D printing technology can make the doctor in the Department of orthopedics intuitive understanding of the knee joint before the operation,accurate guidance for the development of surgical procedures,simulation of the operation,improve the accuracy of the operation. |
