Summaries Of Professor Shao Zhaodi’s Clinical Experiences On The Treatment Of Middle-aged And Elderly Female Urethral Syndrome

Objective:Urethral Syndrome(US)is a common and frequently-occurring disease in middle-aged and elderly female,it has no abnormal laboratory examination indexes,but its recurrent-occurring symptoms such as drench,drain,acerbity or pain,often bring great pain and psychological pressure to patients,seriously affect the quality of their life.Because there is no obvious indication of infection from the point view of Western medicine,so the anti-infection treatment is not only ineffective,but also easily lead to antibiotic resistance,drug allergies,increased diseasesusceptibility to patients and other side effects,other treatments such as auxiliary drugs,surgical procedures,electrical stimulation and so on also can not achieve a very good effect.Therefore,the Traditional Chinese Medicine(TCM)treatment of this disease has great significance and unique advantage.The paper further inherits and carries forward the academic thoughts of Professor Shao Zhaodi on the diagnosis and treatment of kidney disease by summarizing Professor Shao Zhaodi’s clinical experience on the diagnosis and treatment of middle-aged and elderly female urethral syndrome,and then provides a good guidance for academic inheritance and clinical application.Methods:1.Comprehensive collecting and collating Professor Shao Zhaodi’s papers,teaching rounds records,academic lectures and case discussion records and some relevant informations for reflecting the academic thoughts of Professor Shao.2.Select the summary of research in middle-aged and elderly female urethral syndrome as the main line,and combine TCM syndrome differentiation with modern diagnosis and treatment technology,and then do the research by collecting a total of 70 cases of patients with urethral syndrome who were treated in the clinic department of Professor Shao’s and had obvious curative effect from March 2015 to December 2016 systematically,and studying retrospectively the collected clinical cases.Each case includes basic informations about the patient,symptoms and signs at each visit,results of relevant auxiliary examination,disease diagnosis and prescription.The selected cases were female patients over 45 years old,and the clinical symptoms and the results of clean midstream urine culture,urine routine examination are consistent with the diagnostic criteria of urethral syndrome.3.By analyzing the characteristics of diagnosis thoughts,the syndrome differentiation and the prescription of Professor Shao Zhaodi in treating the disease with TCM,Professor Shao’s clinical experiences and academic thoughts of treating this disease were summed up.Results:Professor Shao Zhaodi believes that this disease belongs to "strangury disease" in TCM category,although the etiology and pathogenesis of this disease are complex,and the clinicalmanifestations are different,but the fundamental pathogenesis of the disease is no more than Vital qi deficiency while pathogenic qi prevailing.The Vital qi deficiency is the responsibility of the deficiency of liver,spleen,kidney,and asthenia of both the spleen and kidney is the main reason;the pathogenic qi prevailing contains both exogenous evil and endogenous evil,mainly to pathogenic heat,moisture,blood stasis and qi stagnation.Complexity of the disease condition,prolonged course of disease,including both excess caused by deficiency and deficiency caused by excess,it often presents deficiency in vital qi and excess in pathogenic qi at the same time,coexistence of deficiency and excess,the pathogenic heat,moisture,blood stasis and qi stagnation are not only the pathological products but also the important etiological factors inducing and aggravating the disease.Professor Shao advocates that strengthening body resistance and eliminating evil at the same time,striking a balance between deficiency and excess,treating both the incidental and fundamental aspects,all above should be applied in clinical diagnosis and treatment.Invigorating Spleen and Kidney to strengthen body resistance and support healthy energy;clearing heat and removing dampness to eliminate evil and treat the symptoms;paying attention to regulating the liver and protecting Yin-blood all the time;observing pathogenesis and being good at activating blood circulation,all of that are needed.Clinical syndrome differentiation is mainly divided into four types:deficiency qi of spleen and kidney,defieiency of kidney-Yin,damp heat of bladder,stagnated heat of liver channel.Syndrome which is deficiency qi of spleen and kidney,should to invigorate thespleen and replenish qi,warm kidney and eliminate damp,and give the addition and subtraction of Shen Ling Bai Zhu San;syndrome which is defieiency of kidney-Yin,should to nourish yin and tonify kidney,clear heat and drain dampness,and give the addition and subtraction of Zhi Bo Di Huang Tang;syndrome which is damp heat of bladder,should to clear heat-fire,remove dampness to treat stranguria,and give the addition and subtraction of Ba Zheng San;syndrome which is stagnated heat of liver channel,should to clear liver-fire and promote blood circulation to relieve depression,and give the addition and subtraction of Dan Zhi Xiao Yao San.Professor Shao usually has the placid medication and advocates strengthening body resistance and eliminating evil at the same time on the treatment of the disease.Eliminating evil often use some herbs which are sweat and cold,clearing and mild,such as Ricepaperplant Pith,Plantago seed,Poria cocos,Talc,Alisma and so on,while less using or caution with some herbs which are bitter cold and may spoil one’s stomach,such as Rhubarb,Dandelion etc.Strengthening body resistance and tonifying the deficiency often use some herbs which are mild to tonify both Qi and Blood,Yin and Yang,especially to supplement Qi and nourish Yin,such as Codonopsis pilosula,Fructus corni,Yam,Ligustrum lucidum,Eclipta,Licorice etc,while less using some smooth and shiny herbs like Radix rehmanniae praeparata,Donkey-hide gelatin etc.Besides,Professor Shao also make prescriptions and choose herbs according to the theory of meridian tropism to search for the primary cause of disease in treatment,so that the efficacy of drugs can directly aim to the disease,and the meridian tropism of the selected drugsare mostly spleen and kidney.Because the disease is mostly caused by poor gas machine,gasification disorders,and dampness,heat,blood stasis and turbidity are often mixed up together in later stage,both of them make the disease protracted and difficult to cure,therefore,Professor Shao always use some herbs which are pungent in flavor and warm in nature and good at regulating the Qi and invigorating the circulation of blood throughout the disease treatment,such as Dried tangerine peel,Ligusticum wallichii,Angelica etc.Conclusion:Under the guidance of the wholism concept of TCM and the theory of syndrome differentiation and treatment,Professor Shao Zhaodi follows the treatment principle of strengthening body resistance and eliminating evil at the same time,striking a balance between deficiency and excess,and matching with the life adjustment and maintenance,making a combination of treatment and prevention,by this means,not only the clinical symptoms of patients were significantly alleviated,but also effectively improve the quality of patients’ life.
