Objective To investigate whether theory of mind(To M)obstacle in patients with stable severe Chronic Obstructive Pulmonary Disease(COPD),and to analyze the clinical significance of neuropsychological manifestations in patients with severe stable COPD,to provide a basis for the intervention of To M intervention in patients with COPD.Methods 40 cases of imminent discharged patients with severe stable COPD in Respiratory Medicine,Hefei No.2 People's Hospital(Hefei Hospital Affiliated to Anhui Medical University),from Jan.2015 to Nov.2016,were investigated,among of whom were male 35 cases,female 5 cases,age 58-89 years old.Besides,29 healthy check-up persons who are of the same age and educational level were enrolled as Control Group(male 18 cases,female 11 cases,aged 61-84)and accepted a series of tests—Mimi-mental state examination(MMSE),Hamilton Anxiety Scale(HAMA),Hamilton Rating Scale for Depression(HRSD),faux pas tasks and eye area basic emotion recognition test,and a comparative analysis of test results was made.Arterial blood gas analysis and pulmonary function tests were performed in COPD group.Results There were statistical differences(P<0.05)in the score of COPD group and control group in faux pas tasks.The scores of gaffes recognition and mental state judging were(8.05±1.75)vs.(9.79±0.62)and(16.05±5.18)vs.(25.07±2.98)respectively.There was no statistical differences(P>0.05)in the score of COPD group and control group in the ability of understanding the story on control problem(9.73±0.45)vs.(9.89±0.62).In eye area of basic emotion recognition test,the score of disgust in both groups was(16.48±2.67)vs.(17.69±1.78),the score of fear was(16.05±2.00)vs.(17.03±1.57),sadness was(16.08±2.73)vs.(18.38±1.52),for surprise is(17.10±2.23)vs.(18.83±1.04),and anger was(15.65±2.62)vs.(17.79±1.59)respectively,above all of them existed in statistical differences(P<0.05),while there was no significant statistical difference(P>0.05)in that of joy(19.60±1.08)vs.(19.86±0.58).Pearson correlation analysis showed that there was no significant correlation in the course of faux pas tasks between arterial blood Pa CO2,Pa O2,oxygenation index(Pa O2/Fi O2),Airflow limitation severity(GOLD grade)and gaffes recognition,as well as mental state judging(r=-0.13~0.24,P>0.05).There was no significant correlation in the course of eye area basic emotion recognition tests between arterial blood Pa CO2,Pa O2,Pa O2/Fi O2,GOLD grade and joy,disgust,fear,sadness,surprise,anger(r=-0.22~0.25,P>0.05).Therefore,no correlation exists between the correctness of gaffes recognition,mental state judging,eye area recognition(joy,disgust,fear,sadness,surprise,anger)and changes of arterial blood gas analysis,airflow limitation severity for patients with severe stable COPD.Conclusion To M obstacle existed in patients with severe stable COPD,which were mainly the correct recognition obstacles in gaffes recognition,mental state judging and eye area basic emotions(disgust,fear,sadness,surprise,anger).For the next step to study COPD patients need to provide To M basis for intervention.
