Efficacy Of Yangxue Qingnao Granules For Vascular Cognitive Impairment:a Meta-analysis

Objective: To evaluate the clinical efficacy of Yangxue Qingnao granules on vascular cognitive impairment.Methods: A set of inclusive and exclusive criteria was established.Thorough information retrieval was conducted using the following database:CNKI database,VIP Chinese Technique Magazine database,wan Fang database,Pubmed,Embase and Cochrane library database.The search date ended in November 2016.We chose the randomized controlled trials of Yangxue Qingnao granules on vascular cognitive impairment.After secondary screening of the full articles,selected studies were subjected to the bias risk assess,and relevant data extracted.The data was divided into 5groups:”Yangxue Qingnao granules+Piracetam Tablets+Nimodipine Tablets VS Piracetam Tablets+Nimodipine Tablets”,”Yangxue Qingnao granules+Donepezil Hydrochloride Tablets VS Donepezil Hydrochloride Tablets”,”Yangxue Qingnao granules VS blank control”,”Yangxue Qingnao granules VS Nimodipine Tablets”,”Yangxue Qingnao granules VS Piracetam Tablets”.All groups were evaluated using the following set of criteria:”Effectiveness of treatment”,“MMSE”,“ADL”,“HDS”.All data was analyzed using the RevMan 5.2.Results: A total of 20 studies included 1524 cases of patients,the bias risk were “unclear” almost.Which main performance in the method of randomization is not stated,whether a commitment scheme or blind experiment was used.Below are the results of Meta-analysis after extraction of relevant data:(1)In improving overall efficient of patients with VCI,Yangxue Qingnao granules is superior to Nimodipine Tablets,and combination of Yangxue Qingnao granules with Piracetam Tablets and Nimodipine Tablets is superior to Piracetam Tablets and Nimodipine Tablets,difference were statistically significant.(2)In improving the MMSE score,Yangxue Qingnao granules is superior to Piracetam Tablets,and combination of Yangxue Qingnao granules with Piracetam Tablets and Nimodipine Tablets is superior to Piracetam Tablets and Nimodipine Tablets,difference were statistically significant.Yangxue Qingnao granule is effective to improve the MMSE score,and combination of Yangxue Qingnao granules with Donepezil Hydrochloride Tablets is superior to Donepezil Hydrochloride Tablets,but because of the difference were not statistically significant,so we can’t draw a certain conclusion.(3)In improving the ADL score,Yangxue Qingnao granules is worse to Piracetam Tablets,and combination of Yangxue Qingnao granules with Donepezil Hydrochloride Tablets is worse to Donepezil Hydrochloride Tablets,difference were statistically significant.(4)In safety,the existing data show that Yangxue Qingnao granule in treating of VCI is safe in a certain extent.Conclusion:(1)The efficacy of Yangxue Qingnao granules for vascular cognitive impairment is superior than Nimodipine Tablets,and combination of Yangxue Qingnao granules with Piracetam Tablets and Nimodipine Tablets is superior to Piracetam Tablets and Nimodipine Tablets;(2)The efficacy of Yangxue Qingnao granule on improving intelligence is superior than Piracetam Tablets or Piracetam Tablets with Nimodipine Tablets;(3)Yangxue Qingnao granules is invalid on improving activity of daily living;(4)Yangxue Qingnao granule or Yangxue Qingnao granule with Donepezil Hydrochloride Tablets is effective to improve intelligence,but because of the difference were not statistically significant,so we can’t draw a certain conclusion.Given the research methodological quality is limitation,they should be treated withdialectical view.(5)Yangxue Qingnao granule in treating of VCI is safe in a certain extent.
