The Application Of Ileus Tube In The Early Postoperative Inflammatory Small Bowel Obstruction

Objective: To evaluate the application value of ileus tube for early postoperative inflammatory small bowel obstruction, provide the foundation of choice of the clinical treatment in early postoperative inflammatory small bowel obstruction.Methods: Collect a total of 53 Patients diagnosed with early postoperative inflammatory small bowel obstruction between September 2013 to February 2016. According to the continuous gastrointestinal decompression tube type, patients were divided into the ileus tube group and nasogastric tube group. The same routine therapy was applied to all the cases in the ileus tube group and nasogastric tube group, including abrosia, total parenteral nutrition support and application of antibiotics. To observe the changes of the abdominal distension, the abdominal pain, the abdominal circumference reduction, the average amount of gastrointestinal decompression, the disappearing time of air fluid level, the deceased time of aerofluxus and defecation, the time of the liquid diets, the complication rates, the operability, the average length of stay, the total cost of treatment, the recovery rate and the recurrence rate.To compare the curative effect between ileus tube and nasogastric tube in the early postoperative inflammatory intestinal obstruction.Results: 27 cases were cured by expectant treatment in the ileus tube group, but 2 cases were operation in the ileus tube group; 23 cases in the nasogastric tube group were cured by expectant treatment, and 1 case was operation in the nasogastric tube group. There was no statistical significance on the total cost of treatment, the operability, the recovery rate and the recurrence rates between ileus tube group and nasogastric tube group(P>0.05); the time of abdominal distension and abdominal pain relief(1.97±0.98)d in the ileus tube group was shorter than those(3.08±1.25)d in the nasogastric tube group(P<0.05); the deceased time of aerofluxus(4.93±1.58)d in the ileus tube group was shorter than the time of aerofluxus(6.38±1.71)d in the nasogastric tube group(P<0.05); the deceased time of defecation(8.83±2.07)d in the ileus tube group was shorter than these(10.04±2.07)d in the nasogastric tube group(P<0.05); the ileus tube reduced patient’s abdominal circumference more significant(12.49±2.11)cm versus(7.51±1.05)cm at 72 h after intubation(P<0.05); the disappearing time of liquid and gas in the small bowel was shorter in the ileus tube group(10.21±1.61)d than in the nasogastric tube group(12.88±1.99)d(P<0.05); the ileus tube had a significant effect on draining digestive juice(647.26±166.73)ml/d versus nasogastric tube(502.29±142.51)ml/d(P<0.05); the ileus tube had a significant effect on draining digestive juice(808.74±248.21)ml/d at 72 h after intubation versus nasogastric tube(655.20±107.68)ml/d(P<0.05);the time of the liquid diets(13.93±2.69)d in ileus tube group were faster than those(15.79±1.93)d in the nasogastric tube group(P<0.05); the average length of stay(20.72±8.66)d in ileus tube group were shorter than those(26.04±9.55)d in the nasogastric tube group(P<0.05); but the complication rates(55.2%) were greater than those(20.8%) in the nasogastric tube group(P<0.05).Conclusion: The ileus tube can more effectively to gastrointestinal decompression, relieve the clinical symptoms and signs, it can promote gastrointestinal peristalsis and to accelerate the time of the liquid diets, thus shorten the average length of stay. The ileus tube has notably treatment effect for the patients with early postoperative inflammatory small bowel obstruction than nasogastric tube, but may pay more attention to prevention of water and electrolyte disorder in using ileus tube.
