Objective :To study the dynamic change of blood D- dimer, CRP, platelet, IL- 6,white blood cells in different periods of children with mycoplasma pneumonia and the correlation between each other, and explore the value of D- dimer in evaluation of the severity dge the evaluation of D – dimer in the severity of mycoplasma pneumonia.Results:114 cases of mycoplasma pneumonia at the age from 8 months to 12 years old, and average age was 6 years old. Compare with the mild, severe mycoplasma pneumonia and bacterial pneumonia.F values in analysis of variaof mycoplasma pneumonia.Method:222cases of pneumonia children admitted to our hospital from May 2014 to May 2015 with the method of prospective case-control study. They were divided into three groups including mild mycoplasma pneumonia group(81cases), bacterial pneumonia group(108cases) and severe mycoplasma pneumonia group(33cases)according to the infectious pathogens and disease severity, and the levels of their D-dimer, CRP, blood platelets, IL–6 and white blood cells were studied at 24 hours,72 hours and 120 hours respectively after admission. At the same time, 57 healthy check-up outpatient were selected as control group and the level of D – dimer were evaluated. In the end, junce of peripheral blood D- dimer, CRP, blood platelet, IL-6 and white blood cell after 24 hours admitted to hospital were: 118.32(P<0.05),17.8(P<0.05) and 0.05(P<0.05), 2.15(P>0.05), 0.05(P<0.05), while comparing severe mycoplasma pneumonia group with mild mycoplasma pneumonia group,bacterial pneumonia group and normal control group.The F value after 72 hours admitted to hospital were: 22.72(P<0.05), 1.46(P>0.05), 0.05(P<0.05), 1.7(P>0.05), 0.05(P<0.05); The F value after 120 hours admitted to hospital were: 67.36 (P<0.05), 4.53(P<0.05) and 0.05(P>0.05), 6.3(P<0.05) and 0.05(P<0.05). Level of D- dimer in Group of children with severe mycoplasma pneumonia were highest after 24 hours admitted to hospital when compared with the mild mycoplasma pneumonia group, bacterial pneumonia group and normal control group(P<0.05).Number of white blood cell in bacterial pneumonia group when 24 hours after admitted to hospital has significant difference as compared to the mild mycoplasma pneumonia group and severe mycoplasma pneumonia group(P<0.05).Peripheral blood platelets in bacterial pneumonia group when 24 hours after admitted to hospital has significant difference as compared to the mild mycoplasma pneumonia group and severe mycoplasma pneumonia group(P<0.05).The correlation coefficient of D–dimer and CRP was 0.165 in 24 hours(t=0.014, P<0.05). The correlation coefficient of D–dimer and IL-6 was 0.004 in 24 hours(t=0.955, P>0.05). The correlation coefficient of D–dimer and white blood cells was-0.140 in 24 hours(t=0.037, P<0.05). Logistic regression analysis showed that the significant level of D-dimer and CRP and platelet were 0.000, 0.035 and 0.013respectivel(P<0.05)as the independent factors associated with severe mycoplasma pneumonia. With the severity lever of mycoplasma pneumoniae as the dependent variable, AUC area of D-dimer was 0.981 and the critical value was 485.45ng/ml,with sensitivity ratio of 93.8% and specificity of 98.8%; AUC area of CRP was0.741 and the critical value was 19.755mg/L, with sensitivity ratio of 53.1% and specificity of 91.4%; AUC area of blood platelets was 0.469 and the critical value was159×109/L, with sensitivity ratio of 68.8% and specificity of1.2%; AUC area of IL-6 was 0.732 and the critical value was 6.3pg/ml, with sensitivity ratio of 81.3%and specificity of 63%; AUC area of white blood cells was 0.573 and the critical value was 9.26×109/L, with sensitivity ratio of 68.8% and specificity of 58%.Conclusions D- dimer, CRP, platelet counts were evaluating independent risk factors for severe mycoplasma pneumonia, the three correlation, where D- dimer has higher sensitivity and specific degrees, can be used to evaluate light and severe mycoplasma pneumonia.
