Objective:1.To study variation of the detecting serum levels of PG in patients with stomach diseases.To investigate the relationship of PG and Hp.2. To investgate the clinical significance of serum PGⅠ, the ratio of PGⅠ/PGⅡ(PGR) content and serum CEA, CA19-9 for diagnosis of gastric cancer and precancerous disease.Methods:1. Select a total of 108 patients with gastric diseases,including 25 cases of none atrophic gastritis,23 cases of atrophic gastritis,22 cases of gastric ulcer,38 cases of gastric cancer.2. Serum PGⅠ, PGⅡwere tested by immunoturbidimetry of latex-enhanced in 108 patients with gastric diseases. 14 C urea breath test detection of Hp infection. Serum CEA and CA19-9 levels were tested by the chemiluminescence immunoassay.3. Using SPSS20.0 data application software package for processing. Using chi-square test comparing Hp positive rate and negative rate.Using two-sample t-test comparing content of PG.The diagnosis of gastric cancer were determined by area under the ROC curve threshold.Results:1. The serum PGI content of none atrophic gastritis, atrophic gastritis, gastric cancer respectively are(132.22 ± 72.24)ng/ml,(100.74 ± 35.50)ng/ml,(89.15 ± 53.04)ng/ml; PGR respectively are(6.71±3.15),(4.41±2.18),(4.29±2.34);PGⅡrespectively are(30.65 ± 41.65)ng/ml,(23.84 ± 11.14)ng/ml,(21.12 ± 12.32)ng/ml.Compared with none atrophic gastritis, the content of serum PGⅠand PGR in atrophic gastritis and gastric patients cancer decreased significantly. The differences were statistically significant. But the patients level of serum PG Ⅱ was no statistically significantly different. Compared with the atrophic gastritis patients, the content of PGⅠ, PGR in gastric cancer patients was slightly lower, but the difference was not statistically significant. The level of serum PGI, PGⅡ,PGR in gastric ulcer patients respectively are(203.84±104.38)ng/ml,(36.40±25.34)ng/ml,(6.52±4.12). The level of serum PGI, PGⅡin gastric ulcer patients obviously higher than that of the other three groups, the difference was statistically significant.2. The serum CEA content of none atrophic gastritis, atrophic gastritis, gastric ulcer,gastric cancer respectively are(1.68±0.82) ng/ml,(2.55±1.44)ng/ml,(1.96 ±1.40)ng/ml,(48.63±106.13)ng/ml;The serum CA19-9 content respectively are(9.51±10.88)U/ml,(10.59 ± 7.92)U/ml,(7.80 ± 5.95)U/ml,(148.25 ± 279.28)U/ml. The serum level of CEA and CA19-9 in gastric cancer patients are significantly higher than other three groups, the difference was statistically significant. The other three groups is not statistically significant.By contrast, serum PGⅠ, PGⅡ and PGR content can provide diagnostic value in gastric cancer and precancerous disease.3. Four groups of Hp infection positive rate were 40%, 73.9%, 68.2%, 40% respectively.Hp infection positive rate of atrophic gastritis and gastric cancer patients were significantly higher than the none atrophic gastritis group, the difference was statistically significant. Four groups of Hp infection in patients with positive and negative Hp infection of PGⅠ, PGⅡand PGR value have no statistical significance.4. When the serum level of PGⅠ≤70.05 ng/ml, the sensitivity and specificity for diagnosis of gastric cancer respectively are 88%, 42.1%;when PGR ≤ 3.25,the sensitivity and specificity in diagnosis of gastric cancer respectively are 88%, 36.8%. When PGⅠ≤70.05ng/ml and the PGR≤3.25, the sensitivity and specificity for diagnosis of gastric cancer were 77.4% and 63.4%.Conclusions:1. The reduction of serum PGⅠand PGR were associated with the occurrence and development of gastric cancer. Serum PGⅠ≤70.05 ng/ml and the PGR≤3.25 for the detection of gastric cancer has better sensitivity and specificity. Detection of PGⅠ, PGⅡ and PGR value in gastric cancer and precancerous disease has clinical diagnosis significance.2. The serum levels of CEA and CA19-9 increased significantly in patients with gastric cancer, it has diagnostic value for gastric cancer.The diagnostic value in precancerous disease is not high.3. This experiment groups of Hp infection positive group with negative Hp infection in PG level change are not obvious, due to the small sample size, this study still need further research to explore.
