IntroductionPepsigen(PG) is an inactive precusor of pepsinspecial functional enzyme of gastric mucosa. Human PG can be divided into two sub -groups with different characters of bio - chemistry and immunocompe-tence: PGI and PGII. The level of serum PG can reflect the morphological and functional status of the gastric mucosa, and combined determination of PGI and PGI; II ratio can be called " serologic biopsy" . At present, the main applications of serum PG assay are: 1. The pre -screening of gastric cancer. 2. The evaluation of the diagnosis and treatment of the gastric diseases.The occurrence of the gastric cancer is a multi - steps and multi - factors process. Helicobacter. pylori ( H. p) is recognized as the initiator in the range of the gastric cancer. Studies show that eradication of H. p infection can improve the acute and the chronic gastritis, recover the function of the gastric secretion, decrease the recurrence of the peptic ulcer, cure the MALT and is possible to end the occurrence of the gastric cancer. Hence, its valuable of eradication of H. p for the treatment and prevention of the gastric and duodenal diseases.Nowadays, though methods such as bacteria culture, mucosa biopsy, RUT, C13 and C14 UBT and H. p IgG body etc can be used for judgment of H. p infection and eradication,"theyre not common. So it's more important to find a simple, easy and effective index for H. p infection and eradication.The basic change caused by H. p infection is gastritis, and the level of serum PG is the good index for the mucosa function and status. What on earth is the relationship between both? If H. p infection and eradication do influence the level of serum PG, can serum PG be used for the index of H. p infection and eradication for its advantages such as un - invasion, simplicity, rapidness, high - sensitivity, specificity and high accuracy? This study aims at pursuing a further discussion of the application of serum PG as index for H. p treatment and eradication by analyzing the level of serum PG among crowds and its effect on HG infection and eradication.This study is composed of two parts: I : Compare the serum PG level among different crowds in the Zhuanghe, Liaoning Province. II : on this basis, study the regulation before and after the eradication of serum PG by measuring the level of serum PG before and after eradication of the H. p infection patients, and then test the validity of serum PG as markers for H. p eradication.Part oneMaterials and Method: measure the levels of serum PGI and PGII by IRMA among 1167 cases, and diagnose the gastric diseases by the gastric mucosa histological biopsy. Three methods associated to determining the H. p infection: gastric mucosa dyeing by HE, Hp -DNA PCR and ELISA for Hp IgG antibody.Results: serum PGI decreases with aging, and serum PGII increases with aging and decreases after 60;, as for the ratio of PGI: IIdecreases gradually with aging and increases after 60. What's more, the levels of PGI and PGII in females are obviously higher than those in males. With the severity of gastric mucosa atrophy, the level of serum PG falls little by little, PGI: II ratio with gastric cancer is remarkably lower than the normal, and the level of serum PG with H. p infection is higher than non - infection.Part twoMaterials and Method; to 359 H. p+ patients, eradicate the bacteria by routine triple therapy. Measure the serum PGI and PGII before and 1 month , 5 months and 18 months after treatment by IR-MA. At the same time use three methods determining the result of e-radication therapy.Results: For patients with H. p eradication successfully, one month later serum PGI , PGII levels and PGI; II ration became 82. 04% ,58. 74% and 146. 72% of the levels before treatment respectively, as for five months later they were 73. 14%,63.43% and 130. 67% respectively.
