Clinical Observation On The Therapeutic Effect Of Guipi Decoction Treatment Of Post-stroke Depression (Deficiency Of Both Heart And Spleen)

Objective:This study uses recognized efficacy criteria, diagnostic criteria of depression after stroke(deficiency of two) in the treatment of patients, to explore the effect of Guipi Decoction, and treatment with conventional western medicine as control, aimed at a comprehensive understanding of Guipi Decoction daily life ability in improvements in neurological function, depressive symptoms, and activity of daily living. Method:Selection of post-stroke depression(deficiency of both heart and spleen) patients with 72 cases, were randomly divided into two groups, 36 cases in the control group, the use of fluoxetine(Prozac), daily 20 mg, warm water delivery service; 36 cases in the observation group, the use of Guipi Decoction, 2 times a day sooner or later, warm clothes, daily 1 agent. At the same time, two groups of patients were given conventional therapy, psychological therapy and rehabilitation exercise therapy, two groups were treated with 15 d, TCM syndrome integral scale, Hamilton Depression Scale(HAMD), America National Institutes of Health Stroke Scale(NIHSS), activities of daily living(ADL) scale kinetic energy to evaluate before and after treatment in the two groups were compared; at the same time to detect the adverse reactions of patients after treatment, for the treatment of depression after stroke, comprehensively determine the efficacy and safety of Guipi decoction. Result:According to the results of the evaluation criteria, compared with the treatment, the two groups of patients after treatment of TCM syndrome integral results have different degrees of decline(P<0.05); observation group to improve the effect is better than the control group(P<0.05). Compared with the treatment, the two groups of patients after treatment HAMD scale evaluation results have different degrees of decline(P<0.05); observation group to improve the effect is better than the control group(P<0.05). Compared with the treatment, two groups of patients after treatment NIHSS evaluation results have different degrees of decline(P<0.05); but the two groups are no difference(P>0.05). Compared with before treatment, the two groups of patients after treatment ADL scale evaluation results were decreased(P<0.05); but the two groups were no difference(P>0.05). The evaluation result of TCM syndrome integral effect shows that the total effective rate of the observation group is 88.89%, and the total effective rate is 75% in the control group(P<0.05). The results showed that the total effective rate of the observation group was 88.89%, and the total effective rate was 75% in the control group(HAMD), and the total effective rate was in the two groups(P<0.05). The correlation between the two groups showed that there was a correlation between the TCM syndrome score and the HAMD test(P<0.01) before and after the treatment. The NIHSS and HAMD in the control group were correlated(P<0.05); the ADL and HAMD in the control group were correlated(P< 0.05). Adverse reaction observation showed that the observation group of 3 cases of adverse reactions, adverse reaction rate was 7; the control group of 8.33% cases of adverse reactions, adverse reaction rate was 19.44%; two groups of adverse reaction rate was statistically significant(P<0.05) Conclusion:(1) in post-stroke depression(deficiency of both heart and spleen) treatment, through the use of Guipi Decoction can effectively improve TCM symptoms, neurologic impairment and depressive symptoms and ability of daily life, and the effect is better than western medicine treatment, and has low rate of bad pronunciation is worthy of popularizing.(2) the result of correlation analysis of each table, one shows that the TCM syndrome integral scale and HAMD scale evaluation results have high consistency, we can guide the evaluation of Guipi Decoction from many aspects of treatment effect, on the other hand, shows that the degree of depression and the patients daily life ability, neural function with correlation, suggesting that we should strengthen the early treatment to help patients recover from the nerve function, providing self-care ability of daily life.
