Objective To establish and apply the information management system for fall prevention and control on sanitarium cadres and evaluate the effectiveness. The system is based on Internet of things technology, relies on the professional and high quality resources of hospital, optimizes and integrates the sanitarium medical power. To assess fall status, balance ability, cognitive behavioral, psychological condition and the environment at home of sanitarium cadres. To analyze the related risk factors influential to fall. To evaluate the effectiveness of the remote continuous comprehensive intervention to reduce fall risk.Methods The first part:through evidence-based literature retrieval, experts consultation and preliminary experiment, we discussed the fall comprehensive prevention and control strategy on the sanitarium cadres. Relying on Internet things of technology and information means, we established the information management system for fall prevention and control on sanitarium cadres, which was a collection of risk assessment, remote education and feedback. The second part:use the system to assess the fall status of 126 sanitarium cadres, quantify risk grade and analyze the risk factors related to fall. Screened out 84 high-risk sanitarium cadres, who were involved in the remote continuous comprehensive intervention and analyzed the fall related situation before and after intervention, which evaluated the effectiveness of system.Results The system was stability and in good operation. Assessment, intervention and feedback could run smoothly. Sanitarium cadres generally believed that the system was innovative, convenient, strong interactivity and easy to accept. Sanitarium staff also thought that their professional knowledge and communication ability got promoted, and daily work became more substantial. The sanitarium cadres was 82.88±5.95 years old on average.71.43% was super-aged. Although the incidence of fall was with the domestic low (16.67%). the major (66.67%) belonged to the high risk population of fall. Super-aged, increased frequency of urine, osteoporosis and fear of falling were identified as the main risk of fall with Logistic Regression Analysis (P<0.05).38.89% had a poor body balance and gait stability, and high degree of dysfunction showed high correlation to the fall (P<0.05). Knowledge and attitude related to fall were generally good, however, the proper behavior was not enough,33.33% did not pay attention to prevent falls after taking drugs,61.90% did not replace bathroom mat of aged or damaged in time. Serious fear of falling existed in complex daily activities, such as taking public vehicles (3.98±4.42) and crossing the street lonely (4.53±4.19). Risk factors leading to fall existed in the environment at home, focusing on the floor and the bedroom, which had a low accuracy in scale,66.60% and 56.83% in respectively. After 6 months remote continuous comprehensive intervention, the fall incidence of high-risk sanitarium cadres decreased from 21.43% to 4.76% (P<0.01). The body balance and gait stability improved clearly (P<0.01). The rate of taking proper prevention and control behavior significantly improved, especially in exercising, preventing fall after medication and replacing bathroom mat (P<0.01). They believed in themselves not to fall down with more confidence when taking complex behaviors (P<0.01). The security of environment at home significantly enhanced, such as Non-slip soles, flat floor, using the mat, clean hallway, putting frequently-used items on the side (P<0.05).Conclusion Information management system for fall prevention and control on sanitarium cadres was innovative and convenient. The system could roundly assess the status related to fall, accurately screen out high-risk group and implement the remote continuous comprehensive intervention so that the incident of fall was decrease and the situation of fall was improved. In conclusion, the system is stable and effective, can be further popularization and application as a successful pilot.
