A Preliminary Study On Mental Health For Inpatients In The Department Of Otolaryngology

Objective: To assess the mental status of inpatients in the otorhinolaryngology department by Symptom Check list-90(SCL-90)and Hospital Anxiety and Depression Scale(HADS),in order to provide more evidence for improving the quality of life of patients with otorhinolaryngologic diseases.Methods: Total of 546 patients with otorhinolaryngologic diseases were collected that firstly hospitalized in our hospital otorhinolaryngology from September 2015 to February 2016.After finishing the SCL-90 and HADS scale surveys,questionnaires were retrieved and the data were entered.The data were analyzed using SPSS software package.Categorical data were analyzed by chi-square test,measurement data were analyzed using t test or ANOVA.P <0.05 was considered statistically significant.Results:(1)For 546 patients hospitalized in otolaryngology,the average age was 41.4± 19.1 years,total of 312 man who account for 57.14% were recruited,as well as 234 female,accounting for 42.86%;Professional distributions were falled into students with 75 cases(13.74%),workers with 74 cases(13.55%)and farmers 73 cases(13.37%),respectively..Top five diseases diagnosed as: chronic nasal sinusitis in 73 cases,accounting for 13.37%,65 cases of vocal cord polyp(11.90%),63 cases of chronic tonsillitis(11.54%),nasal septum deviation,38 cases(6.96%),nasal bleeding and nasal trauma,29 cases,accounting for 5.31%,respectively.(2)the total score ofSCL-90 were 137.43 ± 24.45 and average score per item was 1.53 ± 0.27,significantly higher than the national norm(1.44 ± 0.43)(t = 4.55,p < 0.001);the prevalence rate was 15.20%.Somatization scoring(1.45 ± 0.40)higher than the national norm of normal(1.37 ± 0.48),the difference was statistically significant(t = 3.45,p < 0.001).Forced symptom factor score(1.84 ± 0.93)was significantly lower than the national norm(1.62 ± 0.58)(t = 6.25,p < 0.001).The sensitive interpersonal relationship score was(1.63 ± 0.39),compared with the national norm(1.65 ± 0.51),there was no statistically significant difference(t = 0.83,p = 0.83).However,the total scores of male patients and average score per item(14.58 ± 3.62)and(1.67 ± 0.40),respectively,and significantly higher than those of the female patients [(15.03 ± 3.30)and(1.61 ±0.37)](p < 0.05).Depression score(1.61 ± 0.39)is significantly higher than the national norm(1.50 ± 0.59)(t = 4.05,p < 0.05).depression score(1.64 ± 0.41)of male patients is significantly higher than that in women(1.55 ± 0.35)(t = 2.79,p = 2.79).Compared with the national norm(1.39 ± 0.43),otolaryngology hospital patients significantly increased anxiety score(1.61 ± 0.39)(t = 10.39,p< 0.001).Anxiety score(1.65 ± 0.40)in male patients is significantly higher than that in women(1.55 ± 0.35)(t = 2.79,p = 2.79).Hostile factor score(1.58 ± 0.56)in atients was higher than the national norm(1.48 ± 0.56),the difference was statistically significant(t = 3.53,P <0.001).Phobic factor score(1.59 ± 0.50)was significantly higher than the national norm(1.23 ± 0.41)(t = 16.30,p< 0.001).Compared with the national norm of normal(1.43 ± 0.57),paranoid score(1.57 ± 0.52)from otolaryngology hospital patients was significantly increased(t = 4.98,p < 0.001).Psychotic factor score(1.30 ± 0.45)was not different with the national norm of normal(1.29 ± 0.42)(t = 0.46,p = 0.46).(3)HAD scale analysis showed that patients with the doubtful anxiety was of 54 cases,accounting for 9.89%,the patients with definite anxiety symptoms were 137 cases,accounting for 25.09%.The patients with doubtful depression was of 42 people,accounting for 7.69%,depressive symptoms,73 people,accounting for 13.37%.Data analysis results show that the detection rate in male was significantly higher than that in female patients with depression(chi-square = 15.09,p < 0.001);the positive detection rate of depression in students and farmers was higher than those in the workers(chi-square = 6.59,p= 0.037).Conclusion: The top five diseases in patients with otorhinolaryngology diseases were mainly charaterized with chronic rhinosinusitis,vocal cord polyps,chronic tonsillitis,nasal septum deviation,nosebleed and nasal trauma.The analysis of the results of the survey using SCL-90 and HADS indicated that the patients with otorhinolaryngologic diseases revealed obvious mental health problems,thus reduced the quality of life of patients,some aspects in the male were more serious,psychological counseling and treatment were needed for psychological problems.
