Clinical Obsreve The Change Of Using Yiqi Huoxue Tongluo Decoction To Treat Stage Ⅲ And Ⅳ Diabetic Nephropathy Patients With Qi Deficiency And Blood Stasis On Serum ADPN、MDA、SOD

Objective:By observing the changes of serum adiponectin、superoxide dismutase、malondialdehyde of diabetic nephropathy patients of stage Ⅲ and Ⅳ with Qi deficiency and blood stasis syndrome, and the influence of Yiqi Huoxue Tongluo Decoction to the indexes above.To explore the serum level of adiponectin and the intervention effect of Yiqi Huoxue Tongluo Decoction on oxidative stress state in diabetic nephropathy patients with Qi deficiency and blood stasis,thereby to prevents and delays the development of diabetic nephropathy.Methods:70 cases of stage Ⅲ and Ⅳ phase Qi deficiency and blood stasis syndrome of diabetic nephropathy were chosen. The patients are randomly enrolled in treatment group and control group of 35 patients. at the same time DM is not merge kidney disease group and normal control group all 20 cases.The treatment group and the control group are given including controlling diet, blood hypertension、blood sugar and lipid foundation treatments.The treatment group are given Yiqi Huoxue Tongluo Decoction one dose a day on the basis of control group,and the medicine will water decoction, morning and evening each once and 200 ml each time. The two groups do not may use affect the efficacy of medicine.The treatment will be lasted twelve weeks. Laboratory indexes as adiponectin 、 superoxide dismutase 、 malondialdehyde are recorded,Analyzing the differences of adiponectin among the groups and the relationship between adiponectin and other laboratory indexes.Blood samples will be stored before and after the treatment to detect serum adiponectin 、 superoxide dismutase、malondialdehyde.To evaluate changes of syndrome integral of TCM as well.Results:The adiponectin and malondialdehyde level of diabetic nephropathyⅢ、Ⅳpatients with Qi deficiency and blood stasis syndrome were higher than the group of health and DM is not merge kidney disease(P<0.01,P<0.01); But the level of the superoxide dismutase is on the decrease,and were lower than the group of health and DM is not merge kidney disease(P<0.01); Correlation analysis show adiponectin levels were correlated positively with UACR、Scr and superoxide dismutase Level(P<0.01). and were correlated negatively with malondialdehyde(P<0.01). and adiponectin levels were correlated negatively with Glomerular filtration rate in stage Ⅳ diabetic nephropathy(P<0.01).Cure effect of the total effective rate in the treatment group was 85.71%, better than the control group is 65.71%(P<0.01). traditional Chinese medicine Syndrome of clinical efficiency total effective rate in the treatment group was 88.57%, which was significantly higher than the control group’s 68.58%(P<0.01). Serum adiponectin、malondialdehyde were significantly reduced after the treatment in both of the two groups, while treatment group decreased obviously better than the control group(P<0.05).Conclusion:The changes of adiponectin level maybe associate with diabetic nephropathy, adiponectin level is closely related to the oxidative Stress.High oxidative stress state could be observed in stage Ⅲ and phase Ⅳ Qi deficiency and blood stasis syndrome of diabetic nephropathy by the elevated levels of serum adiponectin、malondialdehyde and the declined of the superoxide dismutase. Clinical symptoms can be well treated and urine protein can be decreased in patients who taken Yiqi Huoxue Tongluo Decoction,while the curative effect can also be improved.High oxidative stress state could be ameliorated by effected the levels of serum adiponectin、malondialdehyde、superoxide dismutase.
