Application Of Family Motivational Interviewing On Unhealthy Life Styles Of The Coronary Heart Disease Patients

Objective To describe the current situation of unhealthy life-style such as smoking, alco hol consumption, high salt or high fat diet, as well as lacking of sports of in-patient s with coronary heart disease(CHD) and to investigate the effectiveness of family par ticipated motivational interviewing on unhealthy life-style and quality of life. To expl ore a proper education model for patients with CHD.Methods In this randomized controlled trials, participants were recruited from a hospita l in Tianjin, from November 2013 to October 2014.They were divided into experimen tal and control groups based on random number table. In the control group, patient s received motivational interviewing only, while in the experimental group both patie nts and their family were participated, through telephone intervention after discharg e for 1, 3 and 6 months, researchers evaluated the changes in their unhealthy lifestyle s and indices like blood pressure, abdominal circumference, quality of life between t he two groups. All data are statistical processed by SPSS16.0 software for anal ysis. T test and Chi-square test are used to compare baseline data and differen ces of various kinds of phase behavior change of intervention and control grou ps. The analysis of variance of repeated measurement is used to compare the change trend of outcomes in both groups.Results1. Smoking and alcohol consumption Smoking and drinking change phase of experimental group was higher than con trol group after the discharge.The amount of cigarette reduced than before interv ention. The amount of cigarette of intervention group was less than control gr oup after the discharge for 1 and 3 months. Alcohol consumption was reduced i n both groups after the discharge compared with before intervention. Alcohol co nsumption of intervention group was less than control group(P<0.05).2. High salt and high fat diet and Exercise The change phase of high salt and fat diet of experimental group was better tha n control group at the 3 points of time. The daily intake of salt and fat reduced comp ared with the condition before intervention. Daily salt and fat intake compared betw een two groups after discharge showed that experimental group was less than contro l group(P<0.001).After 3 and 6 months of hospital discharge, the change phase of exercise of experimental group showed better than control group(P<0.05). Beside s, frequency and time of exercise of experimental group was higher than control gro up(P<0.05).3. Clinical Indicators and the quality of life After 6- month, abdominal circumference of experimental group reduced comp ared with the figure before intervention and with comparing between groups experim ental group was found smaller than control group(P<0.05). Systolic pressure of tw o groups reduced after discharge for 1, 3, and 6 months. Furthermore, after 6 month s of discharge, diastolic pressure of two groups reduced statistically(p<0.05). As fo r comparing between the two groups, both systolic pressure and diastolic pressure of e xperimental group showed lower than control group(P<0.05). The findings showed t hat the quality of life of the two groups had a rising trend and with the compare betwe en groups, quality of life of experimental group was found better(p<0.05).Conclusion Health education of motivational interviewing for patients is more acceptabl e. The family participated motivational interviewing compared with only the imple mentation for the patients is more effective in drinking, smoking, high salt, high fat diet, exercising, blood pressure, abdominal circumference and improving the q uality of life. The medical staff could consider applying it to the chronic disea se management of inpatient service.
