Objective:Patients with internal hemorrhoid belongs to spleen-type gas trap (STGT) were treated with Buzhong-Yiqi-Decoction(BYD) with different dose of astragalus to evaluate the efficiency. And the expression of fibulin-3 in the hemorrhoid’tissues were examined to explore the influence of BYD on the fibulin-3. The study provides the theoretical and scientific basis for BYD treating hemorrhoid of STGT and the best dose of astragalus.Methods:1. clinical research:66 patients with internal hemorrhoid of STGT were collected and randomly divided into two groups, the experimental group 1(34)and 2(32). They were treated with BYD for 7days which contains different dose of astragalus (20g for groupl,50g for group2). Then the scores of pretherapy and post-treatment were observed.2. experiment research:select 20 patients(10 from groupl,10 from groupl) who Completed clinical trials and meets the standards, and choose 10 patients with internal hemorrhoid of STGT as control group. Then the prolapsed internal tissues were obtained and the fibulin-3’expression of the tissues were detected by western blot.Result:1. BYD with small dose of astragalus (group 1) showed improvement of clinical symptom like hematochezia, anus bulge and prolapsed compared before and after treatment. The scores before and after treatment of anus bulge, prolapsed and the total syndrome have statistical significance (P<0.05), but there were no significance in hematochezia(P>0.05).2. BYD with large dose of astragalus (group 2) had a better efficiency of clinical symptom like hematochezia, anus bulge and prolapsed compared before and after treatment. The scores before and after treatment of hematochezia, anus bulge, prolapsed and total score have statistical significance (P<0.05).3. After treatment, there has no statistical significance between group 1 and group 2 in the scores of hematochezia, anus bulge and prolapsed (P>0.05), but there has statistical significance in total score (P< 0.05). The group with large dose of astragalus showed a better effect.4. Compared with small-dose of astragalus group, the large group has a higher cure rate in treatment of internal hemorrhoid of STGT, but has no obvious difference(P>0.05).5. Compared with control group, the expression of Fibulin-3 in experimental groups enhanced and the difference was statistically significant (P<0.05).6. Compared group 1 and 2, the Fibulin-3’expression increased with the dosage of astragalus added, but there was no statistically significant(P> 0.05).Conclusion:l.BYD had a good efficiency in treating internal hemorrhoid of STGT and the efficiency improved with the astragalus increased.2.The expression of Fibulin-3’in anal cushion tissue reduce result in tissue elasticity reduce, that causes internal haemorrhoids3. The Fibulin-3’expression of the prolapsed internal tissues in BYD groups enhanced obviously. This confirmed BYD promote the expression of Fibulin-3 in internal hemorrhoid of STGT.4. The Fibulin-3’expression increased with the dosage of astragalus added.5. BYD improved the clinical symptom by increasing the expression of Fibulin-3, which could inhibit the activity of MMP9 and decrease the disintegration of elastic fibers in anal cushion. |
