The Clinical Study Of Baimaiointment Combined With Manipulation In Patients With Spasticity After Stroke

ObjectiveTo investigate the clinical effect on combining Baimai ointment with manipulation for patients with spasticity after stroke. To understand the clinical significance in relieving spasticity after stroke. And to observe an effective treatment of Baimai ointment with manipulation on patients with spasticity for improving the motor function and ADL. Then attempt to provide a safe and therapeutic treatments for patients with spasticity after stroke.MethodsThis study is a part of the "12th Five-Year" scientific and technological support program for the rehabilitation of traditional Chinese medicine for the treatment of spasticity after stroke. This paper refer to rearch for Wang Yafeng, "the Chinese medicine massage combined with modern rehabilitation training on stroke hemiplegia spasticity clinical observation ".Using Stata software to calculate sample size, it concludes that each group sample size for 252 cases, taking into account dropout rate is 20%, then the calculation of income of each group sample size for 302 cases, four groups with a total of 1208 cases, through the total center allocated to our center with 38 cases in each sample, because the topic is about to end. So now take 19 cases for control group,26 cases for treatment group to analyze. This research through the computer software used randomized controlled experiment method to group the study. The patients in the treatment group received Baimai ointment daub spasm muscle、Chinese medicine massage and modern rehabilitation treatment. The patients in the control group received only modern rehabilitation treatment. It will take 4 weeks for treatment and observation. Before treatment and in 2、4weeks after treatment, two groups of patients were assessed by Modified Ashworth, Fugl-Meyer assessment of limb (FMA) and Modified Barthel Index(MBI). At the end of the treatment, we take software packages SPSS20.0 for management and statistical analysis, and comparison and contrast between groupsin。Results1. Baseline comparison:Difference was not significant (P＞0.05) in general information and the scores of Modified Ashworth, FMA, MBI before treatment in two groups of patients.2. Effect comparison in the same group:Within the group, difference was significant (P<0.05) in the scores of Modified Ashworth, FMA, MBI before and after the treatment.3. Effect comparison between the two groups after treatment:After two and four weaks’treament, difference was significant (P＜0.05) in the scores of Modified Ashworth and MBI between the two groups. After 2 weeks treatment, the increase of FMA-U in the treatment group was significantly higher than that in the control group, and the difference was statistically significant (P＜0.05). Treatment with two and four weeks, two groups of FMA and FMA-L score were increased, and the treatment group increased more significantly than the control group, but the two groups compared the difference was not statistically significant (P＞0.05).4. Effect comparison in classification group:By stratified analysis, the therapy which combining Baimai ointment with manipulation is more effective in reducing the spasticity and improving the score of FMA and MBI for patients with shorter course of disease and younger age.5. During the period of this study, only 1 patient who use Baimai ointment with a skin rash, no obvious discomfort after treatment.Conelusion1. This study found that Baimai ointment combine with manipulation could prevent spasticity, and is superior to the pure modern rehabilitation programs.2. The method could also improve the ability and quality of daily living and limb motor function for patients, and is superior to the pure modern rehabilitation programs.3. The therapy is more effective in reducing spasticity for patients with younger age, shorter course of disease. At the same time, the drug safty and little side-effect.
