ObjectiveThe purpose of this paper is to evaluate the illness uncertainty of the family members of elder patients with permanent colostomy, then to explore the influencing factors of illness uncertainty level, furthermore, to provide the theory evidence for the reduction of the illness uncertainty level of elder patients with permanent colostomy and for the improvement of home care quality and for the promotion of physical and psychological recovery of patients.MethodsConvenience sampling was applied to select the object.150 family members of elder patients with permanent colostomy who came from colorectal surgery of one tertiary level first class hospitals of Jinzhou and two tertiary level first class hospitals of Shenyang in December 2013 to December 2015. They were investigated by the structured questionnaires which is consisted of General Information Questionnaire, Mishel Uncertainty In Illness Scale-Family Member Form, Social Support Rating Scale and Simplified Coping Style Questionnaire. The data were analyzed by the SPSS 17.0, and P <0.05 was considered statistically significant.ResultsThe total average score of illness uncertainty among family members of elder patients with permanent colostomy was(72.55±9.91), and the average score of four dimensions was uncertainty(3.18), unpredictability(3.15),complexity(2.68), inconsistency(2.35) from high to low.The factors that affect uncertainty in illness of family members of elder patients with colostomy was marital status, the level of education, economic conditions, the level of understanding of family members in colostomy, the relationship with patient and the level of accepting of patients in colostomy.The social support of family members of elder patients with permanent colostomy was negatively associated with the illness uncertainty(r=-0.456, P<0.01). Positive coping of family members of elder patients with permanent colostomy was negatively associated with the illness uncertainty(r=-0.352, P<0.01). Negative coping of family members of elder patients with permanent colostomy was positively associated with the illness uncertainty(r=0.350, P<0.01).Multiple stepwise regression analysis showed that “social support”, “the degree of education”, “the level of accepting of patients in colostomy”, “the level of understanding of family members in colostomy”, “economic conditions” and“negative coping style” were entered model of illness uncertainty of family members of elder patients with permanent colostomy.ConclusionsThe illness uncertainty of family members of elder patients with permanent colostomy was mostly in higher levels, So we should pay attention to illness uncertainty, and take effective measures to reduce the level of illness uncertainty.Social support, the degree of education, the level of accepting of patients in colostomy, the level of understanding of family members in colostomy,economic conditions, and negative coping style were the influence factors of the illness uncertainty of family members of elder patients with permanent colostomy.When providing the care guide to family members of elder patients withpermanent colostomy, we should based on the level of education, economic conditions, the relationship with patients, the level of understanding of family members in colostomy and the level of accepting of patients in colostomy.Improving the social support of family members of patients and encouraging them to take an active coping in the facing of disease progression and prognosis, can effectively reduce the illness uncertainty of family members of elder patients with permanent colostomy.
